目的分析农村主要慢性病患者服药依从性影响因素,探讨基层医生对慢性病患者服药依从性的作用,为提高慢性病患者服药依从性提供参考。方法随机抽取黑龙江省4个县的农村居民1 064人,对其进行问卷调查,对其中181例慢性病患者运用χ2检验、一般描述性分析、多因素logistic回归进行服药依从性影响因素分析。结果具有良好服药依从性的患者68例,占慢病患者的37.6%,建立健康档案、村医督促服药、定期测量血压、村医给予饮食与运动指导、健康信念、慢性病知识影响服药依从性,差异均有统计学意义(P<0.05);多因素logistic回归结果显示,慢性病患者服药依从性的影响因素为基层卫生服务利用(OR=3.574,95%CI:1.360~9.391)与健康信念(OR=2.884,95%CI:1.015~8.196)。结论农村慢性病患者总体服药依从性不高,基层卫生服务利用、健康信念是患者服药依从性的保护因素,应加强基层卫生服务人员对慢性病患者的服务管理,强化患者健康信念,普及慢性病知识以有效提高患者服药依从性。
Objective To analyze the influential factors of medication compliance in rural patients with chronic diseases and to explore the role of grassroots doctors in compliance with medication in patients with chronic diseases and to provide references for improving compliance of patients with chronic diseases. Methods A total of 1 064 rural residents were randomly selected from 4 counties in Heilongjiang Province. A questionnaire survey was conducted among them. Among them, 181 cases of chronic diseases were analyzed by χ2 test, general descriptive analysis and multivariate logistic regression analysis. Results 68 patients with good medication adherence, accounting for 37.6% of patients with chronic disease, the establishment of health records, the village doctor supervise the medication, regular measurement of blood pressure, village doctors give diet and exercise guidance, health beliefs, knowledge of chronic disease medication compliance, (OR = 3.574, 95% CI: 1.360 ~ 9.391) and healthy beliefs (OR: 3.574, 95% CI: 1.360 ~ 9.391) in the chronic obese patients. The differences were statistically significant = 2.884, 95% CI: 1.015 ~ 8.196). Conclusion The overall medication compliance of patients with chronic diseases in rural areas is not high. The utilization of primary health care services and health beliefs are the protective factors of patients ’medication compliance. It is imperative to strengthen the service management of patients with chronic diseases by primary health care workers, strengthen the patients’ health beliefs and popularize the knowledge about chronic diseases. Improve patient compliance with medication.
