目的探讨血清游离轻链测定在系统性红斑狼疮(SLE)活动期中的应用价值。方法根据SLEDAI积分将92例SLE患者分为活动期组和非活动期组,检测其血清κ、λ轻链、抗核抗体(ANA)、dsDNA、IgG、IgM、IgA、C3、C4浓度,并与80健康对照进行比较分析。同时进行SLE活动期组ANA滴度和血清游离κ、λ轻链相关性分析。结果 SLE组血清游离κ、λ轻链明显高于健康对照组,SLE组中活动期患者血清游离κ、λ轻链明显高于非活动期患者,差异均有统计学意义(P<0.01)。相关性统计分析显示κ、λ轻链浓度与ANA滴度呈正相关(r=0.993和r=0.983,P<0.05)。结论血清κ、λ游离轻链在诊断SLE活动期有比较高的临床诊断价值。
Objective To investigate the value of serum free light chain assay in the active stage of systemic lupus erythematosus (SLE). Methods According to the SLEDAI score, 92 patients with SLE were divided into active group and inactive group. The serum levels of κ, λ light chain, antinuclear antibody (ANA), dsDNA, IgG, IgM, IgA, C3 and C4 Comparative analysis with 80 healthy controls. At the same time, the relationship between ANA titer and serum free κ and λ light chain in SLE active stage was analyzed. Results The serum free κ and λ light chains in SLE group were significantly higher than those in healthy control group. The serum free κ and λ light chain in active SLE group were significantly higher than those in inactive SLE group (P <0.01). Correlation statistical analysis showed that there was a positive correlation between the kappa, lambda light chain and ANA titer (r = 0.993 and r = 0.983, P <0.05). Conclusion Serum κ, λ free light chain in the diagnosis of SLE activity has a relatively high clinical diagnostic value.
