杜××男45岁1983年8月5日初诊。主诉双眼阵发性视物模糊,约半年。1982年年初开始,双眼视力有时模糊,伴有间歇性头痛,双眼眶疼。1年来会阴部常发生溃疡,全身皮肤经常起红斑。口腔经常长溃疡约10多年。1982年3月在某医院诊断为“白塞氏病”和“视乳头水肿”。在某医院神经外科检查(包括CT扫描)均属正常。曾服用激素和双氢克尿塞,持续用药约1年。自觉症状无明显好转。半年来,视物模糊发作频繁,每次发作约数秒钟至20～30秒钟。双眼交替发作或同时发作。发病时,眼前不闪光,也不头疼。初诊印象为“双视乳头水 Du XX male 45 years old August 5, 1983 newly diagnosed.
