目的应用MR扩散加权成像(DWI)前瞻性研究其对脑实质囊虫病亚临床期的诊断价值。方法根据制定的纳入标准,筛选110例脑实质囊虫病(亚临床期)患者为研究对象,并与130例脑实质囊虫病活虫期对照,进行MR DWI检查,同时以表观扩散系数(ADC)值和指数表观扩散系数(eADC)值作为观测指标进行对照评价。结果脑实质囊虫病亚临床期组,正常对照镜像区组及活虫期组等各组间经统计学分析评价,差异有统计学意义,脑实质囊虫病亚临床期ADC值高于正常对照镜像区组,低于活虫期组;eADC值低于正常对照镜像区组,高于活虫期组,并应用受试者工作特征(ROC)曲线分析评价ADC值和eADC值对脑实质囊虫病亚临床期与活虫期鉴别诊断效能。结论揭示脑实质囊虫病亚临床期和活虫期病灶DWI水分子运动的特征,制定出相应的ADC值及eADC值诊断指标,并应用于亚临床期脑实质囊虫病的早期诊断,对于及早治疗具有非常重要的指导意义。
Objective To investigate the clinical value of MR diffusion-weighted imaging (DWI) in the diagnosis of cerebral parenchymal cysticercosis. Methods One hundred and ten patients with cerebral parenchymal cysticercosis (subclinical stage) were screened out according to the established inclusion criteria. MR DWI examination was performed in comparison with 130 cases of cerebral cysticercosis live-stage, and the apparent diffusion coefficient (ADC) values and exponential apparent diffusion coefficient (eADC) values were used as control indicators for evaluation. Results The cerebral parenchymal cysticercosis subclinical group, the normal control group and the live control group were statistically evaluated. The ADC value of subclinical cerebral cystosis was higher than normal The eICC values in the control group were lower than those in the live-worms group. The eADC values were lower than those in the normal control group and higher than those in the live-worms group. The ROC curve was used to evaluate the effect of ADC value and eADC value on brain parenchyma Differential Diagnosis of Cysticercosis between Subclinical and Lived Periods. Conclusion The characteristics of DWI water molecules in subclinical and live-stage lesions of cerebral parenchyma were revealed, and the corresponding ADC values and eADC values were established. The diagnostic results were applied to the early diagnosis of subclinical cerebral parenchymal cysticercosis. Early treatment has a very important guiding significance.
