目的:探讨前牙种植患者术前的精神因素对术后疼痛及满意度的影响。方法:随机选取2013年3月至2015年6月于重庆医科大学附属口腔医院种植科就诊的前牙缺失患者,符合纳入标准自愿选择种植义齿修复前牙缺失的患者92例,术前对其进行症状自评量表(symptom checklist-90,SCL90)和状态-特质焦虑量表(state-trait anxiety inventory,STAI)测评,术后进行痛视觉模拟评分(visual analogue scale,VAS)及自我满意疗效评分,测试每位受试者的精神心理状况,并按患者焦虑程度分组与术后疼痛评分和自我满意评分进行Pearson相关性分析。结果:纳入的前牙缺失患者SCL90总分高于全国常模;单项目分析中,前牙缺失患者与全国常模SCL90单项平均分比较,除焦虑项目得分高出12.9%外,其余项目得分与全国常模差异不明显。按量表得分相关标准分组,不同状态焦虑量表(state anxiety inventory,SAI)分组之间的术后疼痛评分和自我满意评分差异都具有统计学意义(P<0.05);不同特质焦虑量表(trait anxiety inventory,TAI)分组之间的术后疼痛评分和自我满意评分差异都具有统计学意义(P<0.05),且趋势与SAI一致。种植术前SAI、TAI和焦虑(SCL90)评分之间均有显著的正相关关系,SAI评分与术后疼痛评分(R=0.471,P=0.024)、自我满意(R=0.492,P=0.031)之间存在相关性;TAI评分同样与术后疼痛评分(R=0.528,P=0.041)、自我满意(R=0.573,P=0.047)之间存在相关性。结论:前牙缺失患者种植术前的精神因素与术后疼痛及满意度之间具有一定的相关性,以后研究应该综合必要的影响因素来分析种植修复患者的精神因素,寻找有无体内物质变化,寻找易于推行的干预措施,将有助于加强医患沟通,提高前牙种植修复的临床诊疗质量。
Objective: To investigate the effect of preoperative mental factors on postoperative pain and satisfaction in anterior dental implant patients. Methods: From March 2013 to June 2015, randomly selected from the Department of Stomatology, Affiliated Stomatological Hospital of Chongqing Medical University, patients with missing anterior teeth, in line with the inclusion criteria of voluntary choice of implant denture repair 92 cases of anterior teeth missing patients before surgery The symptom checklist-90 (SCL90) and state-trait anxiety inventory (STAI) were evaluated. There were significant differences in postoperative pain scores and self-satisfaction scores between groups with different state anxiety inventory (SAI) (P <0.05), while those with different trait anxiety scales traumatic anxiety inventory (TAI), postoperative pain score and self-satisfaction score were all statistically significant (P <0.05), and the trend was consistent with SAI. There was a significant positive correlation between SAI, TAI and anxiety (SCL90) scores before and after operation.
