目的调查甘肃省儿童医院连续11年先天性心脏病住院患儿地域分布及医疗支付方式,为制定先心病患儿诊治策略提供依据。方法收集甘肃省儿童医院2001~2011年先天性心脏病住院患儿的完整病历,提取住院患儿地域分布、年份等基本信息。采用SPSS 16.0软件进行统计分析。结果共纳入591例先心病患儿。患儿来源以陇中地区为主,占68.87%;陇东地区与河西地区就医患儿有逐年增加的趋势,平均增速分别为189.42%和27.62%。经济Ⅲ类地区住院患儿最多,占35.64%,增速最快,平均增速为203.45%。治疗费用2005年前以自费为主,2005年后健康保险和各类社会援助不断增加。首诊机构以县级医疗机构为主,占55.50%,平均增速为170.18%。结论甘肃省经济欠发达地区小儿先心病就医需求逐年增加,由政府主导构建与完善经济落后地区先心病的健康保健制度是先心病人群防治的基本保证。
Objective To investigate the geographical distribution and medical payment of children with congenital heart disease hospitalized in Children’s Hospital of Gansu Province for 11 consecutive years, and to provide basis for making diagnosis and treatment strategies for children with congenital heart disease. Methods The complete medical records of children hospitalized with congenital heart disease from 2001 to 2011 in Children’s Hospital of Gansu Province were collected, and the basic information such as geographical distribution and year of hospitalized children were extracted. Using SPSS 16.0 software for statistical analysis. Results A total of 591 children with CHD were enrolled. The main source of children was Longzhong, accounting for 68.87% of the total. Children in eastern Gansu and Hexi areas increased year by year with average growth rates of 189.42% and 27.62% respectively. In the economy category III, the largest number of children were hospitalized, accounting for 35.64% of the total, the fastest growth rate with an average growth rate of 203.45%. The cost of treatment was mainly paid for by 2005 at its own expense. After 2005, health insurance and various types of social assistance have continuously increased. The first visit to the county-level medical institutions, accounting for 55.50%, an average growth rate of 170.18%. Conclusion The demand for pediatric congenital heart disease in Gansu economically underdeveloped areas is increasing year by year. The government-led construction and improvement of the health care system for congenital heart disease in economically underdeveloped areas is the basic guarantee for the prevention and treatment of congenital heart disease.
