目的探讨无先兆偏头痛(MO)的危险因素。方法使用现场问卷调查的方法 ,以天津医科大学总医院、天津中医药大学第二附属医院2004年11月—2006年3月确诊的MO患者为病例组,按性别、年龄±3岁选取医院其他科室的非MO患者作为对照组,进行1:2匹配的病例对照研究。结果单因素分析结果表明,体力劳动者(OR=0.180,95%CI:0.082～0.396)、体质指数(BMI)高(OR=4.177,95%CI:1.978～8.819)、偏头痛家族史(OR=6.500,95%CI:2.119～19.935)、脑血管病家族史(OR=4.357,95%CI:2.1470～8.8420)、睡眠质量(OR=0.411,95%CI:0.256～0.661)、喜甜食(OR=6.488,95%CI:2.651~15.74)、经常性内心情感体验(OR=12.691,95%CI:4.435～36.318)及工作和生活上的不良应激事件(OR=6.667,95%CI:1.835～24.224)与MO的关联有统计学意义(P<0.05)。多因素分析结果显示,体力劳动(OR=0.143,95%CI:0.034～0.601)、BMI(OR=5.226,95%CI:1.350～20.229)、睡眠质量(OR=0.295,95%CI:0.124～0.701)及有经常性内心情感体验(OR=9.897,95%CI:2.080～47.091)与MO的关联有统计学意义(P<0.05)。结论 MD的发生是多因素综合作用的结果 。
Objective To investigate the risk factors of migraine without aura (MO). Methods Using the method of on-the-spot questionnaire, MO patients diagnosed by Tianjin Medical University General Hospital, Second Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from November 2004 to March 2006 as the case group, the hospital was selected by sex and age ± 3 years Department of non-MO patients as a control group, a 1: 2 matching case-control study. Results The results of univariate analysis showed that the family history of migraine (OR = 0.180,95% CI: 0.082-0.396), body mass index (BMI) was higher (OR = 4.177, 95% CI 1.978-8.819) = 6.500, 95% CI: 2.119-19.935), family history of cerebrovascular disease (OR = 4.357,95% CI: 2.1470-8.8420), sleep quality (OR = 0.411,95% CI: 0.256-0.661) OR = 6.488,95% CI: 2.651 ~ 15.74), frequent emotional experiences (OR = 6.671, 95% CI: 4.435-36.318) and adverse events at work and in life 1.835 ~ 24.224) associated with MO was statistically significant (P <0.05). The results of multivariate analysis showed that the physical quality of sleep (OR = 0.143, 95% CI: 0.034-0.601), BMI (OR = 5.226,95% CI: 1.350-20.229) 0.701), and regular emotional experience (OR = 9.897, 95% CI: 2.080 ~ 47.091) were associated with MO (P <0.05). Conclusion The occurrence of MD is the result of multifactorial combination.
