目的:观察不伴鼻息肉性慢性鼻-鼻窦炎(CRSsNP)的病理学特点,探讨肥大细胞及活化肥大细胞在CRSsNP中的作用及其与嗜酸粒细胞炎症的关系。方法:采用苏木精-伊红染色对CRSsNP和对照下鼻甲组织学特点进行观察以及对组织总炎性细胞、单个核细胞、浆细胞及嗜酸粒细胞进行计数分析。利用甲苯胺蓝和类胰蛋白酶免疫组织化学法分别检测肥大细胞和活化肥大细胞在CRSsNP组和对照组中的浸润情况,并探讨其与组织嗜酸粒细胞炎症的相关性。结果:CRSsNP组织中总炎性细胞、单个核细胞以及浆细胞数均高于对照组,而嗜酸粒细胞和黏膜固有层腺体数量于二者无明显差异。肥大细胞和活化肥大细胞计数呈相关性,但二者在CRSsNP组和对照组中均未见明显差异,且二者与组织嗜酸粒细胞炎症均无相关性。结论:CRSsNP较正常对照具有更重的炎症反应,但肥大细胞及其活化以及组织嗜酸粒细胞二者无差异,且肥大细胞及其活化与嗜酸粒细胞计数无相关性,提示肥大细胞及其介导的嗜酸粒细胞炎症在CRSsNP的发病中并不起重要作用。
Objective: To observe the pathological features of non-nasal polyps chronic rhinosinusitis (CRSsNP) and to explore the role of mast cells and activated mast cells in CRSsNP and its relationship with eosinophilic inflammation. Methods: Hematoxylin-eosin staining of CRSsNP and control under the turbinate histological features were observed and the total inflammatory cells, mononuclear cells, plasma cells and eosinophils count analysis. The infiltration of mast cells and activated mast cells in CRSsNP group and control group were detected by toluidine blue and tryptase immunohistochemistry, and their correlation with tissue eosinophilic inflammation was also investigated. Results: The total number of inflammatory cells, mononuclear cells and plasma cells in CRSsNP tissues were higher than those in control group, while there was no significant difference in the numbers of eosinophils and mucosal lamina propria between the two groups. Mast cells and activated mast cell counts were correlated, but both in the CRSsNP group and the control group showed no significant difference, and both with tissue eosinophil inflammation no correlation. CONCLUSIONS: CRSsNP has a more severe inflammatory response than normal controls, but there is no difference between mast cells and their activation and eosinophils, and there is no correlation between mast cells and their activation and eosinophil count, suggesting that mast cells and Its mediated eosinophilic inflammation does not play an important role in the pathogenesis of CRSsNP.
