目的分析广东省2012年就业流动人口发生伤害的医疗负担。方法 2012年采用多阶段分层整群随机抽样方法,在全省选取了12个流动人口专题调查点,对抽取的制造业、批发零售业、住宿餐饮业、社会服务业、建筑业和其他行业的18~59岁就业流动人口进行问卷调查,调查内容包括伤害患者的伤害原因、门诊和住院就诊情况、医疗费用、住院时间、休息时间等。结果 4 035名调查对象中262人曾发生1次及以上伤害,伤害发生率为6.5%,标化率为6.7%;共发生314次伤害事件,伤害人次发生率为7.78%,标化率为8.16%。发生伤害者人次就诊率为32.7%(86/263),其中门诊就诊率为27.4%(72/263),住院就诊率为5.3%(14/263);不同伤害原因的人次就诊率差异有统计学意义(P<0.01)。门诊就诊伤害患者次均医疗费用为745元,住院就诊伤害患者次均医疗费用为13 258元。不同伤害原因的就诊费用中位数由高到低依次为道路交通伤害(4 000元)、其他(1 000元)、钝器伤(550元)、烧烫伤(385元)、跌倒/坠落(300元)、刀/锐器伤(300元)。结论广东省流动人口每年因伤害就诊导致的医疗负担巨大,应加强伤害的预防和控制工作。
Objective To analyze the medical burden of injury to employment floating population in Guangdong Province in 2012. Methods A multi-stage stratified cluster random sampling method was adopted in 2012, and 12 special floating population survey points were selected in the whole province. The survey of manufacturing, wholesale and retail, accommodation and catering, social services, construction and other industries Of the 18 to 59-year-old employment floating population to conduct a questionnaire survey, the investigation includes injury patients cause of injury, outpatient and inpatient visits, medical expenses, hospital stay, rest and so on. Results 262 out of 4 035 subjects had one or more injuries, the incidence of injury was 6.5% and the standardization rate was 6.7%. A total of 314 injuries were recorded, the incidence of injuries was 7.78%, the standardization rate was 8.16%. The number of visits was 32.7% (86/263), of which 27.4% (72/263) were outpatients and 5.3% (14/263) were hospitalized. The differences in the number of visits among different causes of injury were statistically significant Significance (P <0.01). Outpatient visits hurt the average cost of medical care for patients 745 yuan, the average cost of hospitalization injured patients was 13,258 yuan. The median attendance rates for different causes of injury were road traffic injuries ($ 4,000), others ($ 1,000), blunt injuries ($ 550), burns ($ 385), falls / falls 300 yuan), knife / sharp injury (300 yuan). Conclusion The migrant workers in Guangdong Province have huge medical burdens due to their injuries every year. Injury prevention and control should be strengthened.
