目的观察睫状体冷凝联合周边视网膜冷凝治疗晚期新生血管性青光眼的临床疗效。方法回顾分析了药物不能控制或已行其他抗青光眼手术失败而接受睫状体冷凝联合周边视网膜冷凝手术治疗的新生血管性青光眼48例(48眼)。观察眼压、视力、虹膜新生血管改变及术后并发症。结果 28眼眼压<21mm Hg,19眼联合降眼压药物点眼眼压<30mm Hg。虹膜新生血管完全消退31眼(64.58%),新生血管较术前明显减少17眼(35.42%)。术后视力提高14眼(29.17%),视力不变27眼(56.25%),视力下降7眼(14.58%)。术后并发症:早期多数出现一过性眼压升高,出现眼球萎缩1例。结论睫状体冷凝联合周边视网膜冷凝能有效降低眼压,使部分患者得以保留原有视力,少数有所提高,操作简单,易于掌握,是一种较为安全有效的手术方法。
Objective To observe the clinical efficacy of ciliary body condensation combined with peripheral retinal condensation in the treatment of advanced neovascular glaucoma. Methods Forty-eight patients (48 eyes) with neovascular glaucoma who were uncontrollable or underwent surgery for other anti-glaucoma drugs and were treated with ciliary body condensation and peripheral retinal condensation were retrospectively analyzed. Intraocular pressure, visual acuity, iris neovascularization and postoperative complications were observed. Results 28 eyes intraocular pressure <21mm Hg, 19 eyes with intraocular pressure lowering intraocular pressure eye pressure <30mm Hg. The number of iris neovascularization completely disappeared in 31 eyes (64.58%), and the number of neovascularization was significantly reduced in 17 eyes (35.42%) compared with that before operation. Postoperative visual acuity improved in 14 eyes (29.17%), vision unchanged in 27 eyes (56.25%), vision loss in 7 eyes (14.58%). Postoperative complications: the majority of early transient elevated intraocular pressure, there was 1 case of atrophy of the eye. Conclusion Ciliary condensation and peripheral retinal condensation can effectively reduce intraocular pressure, so that some patients can retain the original visual acuity, a few have increased, simple operation, easy to grasp, is a more safe and effective surgical methods.
