目的比较检测淋巴结阴性乳腺癌患者的血清抗酒石酸酸性磷酸酶5b(TRACP5b)和癌胚抗原(CEA)、糖类抗原-153(CA-153)水平在预测其骨转移方面的临床价值。方法采用酶联免疫吸附法(ELISA)检测77例淋巴结阴性乳腺癌患者和33例健康女性的血清TRACP5b水平;采用电化学发光免疫分析法(ECLIA)检测两组人群的血清CEA、CA-153水平,对两组人群各指标之间的差异进行比较,对结果进行统计学分析。结果乳腺癌组3个血清指标水平相比于健康对照组均明显升高,差异均具有统计学意义(t值分别为-10.583、-3.958和-3.766,均P<0.05);在敏感性方面,血清TRACP5b较高,与CEA、CA-153比较,差异均具有统计学意义(χ~2值分别为19.452、15.845,均P<0.05),特异性方面差异均无统计学意义(χ~2值分别为1.015、2.062均P>0.05);CEA与CA-153相比,敏感性和特异性差异均无统计学意义(χ~2值分别为0.252、0.349,均P>0.05)。结论血清TRACP5b在淋巴结阴性乳腺癌中的敏感性较高,可作为预测淋巴结阴性乳腺癌骨转移的血清学标记物之一。
Objective To compare the serum levels of tartrate-resistant acid phosphatase 5b (TRACP5b) and carcinoembryonic antigen (CEA) and carbohydrate antigen -153 (CA-153) in predicting bone metastases in patients with lymph node-negative breast cancer. Methods Serum levels of TRACP5b in 77 patients with lymph node-negative breast cancer and 33 healthy women were detected by enzyme-linked immunosorbent assay (ELISA). The levels of CEA and CA-153 in serum of two groups were detected by ECLIA , The differences between the two groups of various indicators were compared, the results were statistically analyzed. Results The levels of three serum markers in breast cancer patients were significantly higher than those in healthy controls (t = -10.583, -3.958 and -3.766, respectively, P <0.05). In terms of sensitivity , Serum TRACP5b was higher, the difference was statistically significant compared with CEA and CA-153 (χ ~ 2 values were 19.452 and 15.845 respectively, all P <0.05), but there was no significant difference in specificity (χ ~ 2 Values were 1.015, 2.062, respectively, P> 0.05). There was no significant difference in the sensitivity and specificity between CEA and CA-153 (χ ~ 2 values were 0.252,0.349, all P> 0.05). Conclusion Serum TRACP5b is highly sensitive in lymph node-negative breast cancer and may be used as one of serological markers for predicting bone metastasis of lymph node-negative breast cancer.
