目的:探讨糖化血红蛋白(HbA1c)在妊娠期糖尿病(GDM)早期诊断中的作用和意义。方法:选择长春市人民医院就诊的113例患者,其中糖耐量正常妊娠组(NGT)50例,妊娠糖耐量减低组(GIGT)31例,妊娠期糖尿病组(GDM)32例,采用糖化血红蛋白(HbA1c)法、空腹血糖(FPG)、GCT(50 g葡萄糖负荷试验)或OGTT(75 g糖耐量试验)3种方法进行检查。结果:GDM组的HbA1c的检测结果和阳性率均比NGT组、GIGT组高,具有统计学差异(P<0.05)。在GDM诊断中,HbA1c的敏感性、诊断符合率最高。结论:HbA1c检测法对早期诊断GDM具有重要意义,早期干预治疗可减少母婴发病率及死亡率。HbA1c是临床上筛查、诊断、监测妊娠期糖尿病简单、可靠的方法。
Objective: To investigate the role and significance of glycosylated hemoglobin (HbA1c) in the early diagnosis of gestational diabetes mellitus (GDM). Methods: A total of 113 patients were selected from Changchun People’s Hospital. Among them, 50 were normal glucose tolerance group (NGT), 31 with gestational impaired glucose tolerance (GIGT) group and 32 with gestational diabetes mellitus (GDM). Hypoglycemic hemoglobin HbA1c), fasting plasma glucose (FPG), GCT (50 g glucose load test) or OGTT (75 g glucose tolerance test). Results: The results of HbA1c and the positive rate in GDM group were higher than that in NGT group and GIGT group (P <0.05). In the diagnosis of GDM, HbA1c sensitivity, the highest diagnostic accuracy. Conclusion: The detection of HbA1c is of great significance for the early diagnosis of GDM. Early intervention can reduce the morbidity and mortality of maternal and infant. HbA1c is a simple and reliable method for clinical screening, diagnosis and monitoring of gestational diabetes.
