目的:运用理性预期理论分析试点医院实施临床路径管理的动力来源。方法:利用干预研究法和现场调研法对试点医院积极实施临床路径管理的动力进行全面剖析。结果:重庆市黔江中心医院的案例提示,其实施临床路径管理的动力来自于医院和医务人员对改革的期盼,合理的补偿机制以及透明的改革方案。结论:让医院和医务人员对临床路径实施所涉及的社会、患者、医院以及医务人员自身经济利益等方面产生理性预期,可以积极促进临床路径管理工作的开展。
Objective: To use rational expectation theory to analyze the power source of clinical path management in pilot hospitals. Methods: Using intervention research method and on-the-spot investigation method to conduct a comprehensive analysis of the motivation of the pilot hospitals to actively implement clinical path management. Results: The cases of Qianjiang Central Hospital in Chongqing suggest that the driving force behind the implementation of clinical path management came from expectations of hospitals and medical staffs for reform, a reasonable compensation mechanism and a transparent reform program. Conclusion: It is reasonable for hospitals and medical staffs to make rational expectations about the social, patient, hospital and medical personnel ’s own economic interests involved in the implementation of clinical pathways and can actively promote the development of clinical path management.
