目的观察中西药联合治疗幽门螺杆菌(Hp)相关隆起糜烂性胃炎的临床疗效。方法将110例Hp相关隆起糜烂性胃炎患者随机分为2组。2组均先予抗Hp三联疗法(奥美拉唑20mg、阿莫西林1.0g、呋喃唑酮0.1g,每日2次口服)治疗1周。1周后治疗组56例予奥美拉唑20mg/次每日2次、龙血竭胶囊4粒/次每日3次,饭前半小时口服;替普瑞酮50mg/次每日3次,饭后半小时口服。对照组54例予奥美拉唑20mg/次每日2次,饭前半小时口服。2组均4周为1疗程,停药1月后判断疗效,复查胃镜、检测Hp及胃黏膜组织病理学检查,并进行胃镜下疗效评分和胃黏膜炎症程度评分。结果 2组治疗前后各症状评分均降低,与治疗前比较差异有统计学意义(P均<0.01);且治疗组胃脘痛、饱胀、烧心、泛酸、嗳气的改善情况优于对照组(P均<0.05)。治疗组、对照组总有效率分别为85.8%、55.6%,2组比较差异有统计学意义(P<0.01)。治疗组Hp根除率83.9%,对照组77.8%,2组比较差异无统计学意义(P>0.05)。结论奥美拉唑、替普瑞酮、龙血竭联合抗Hp三联疗法治疗Hp相关隆起糜烂性胃炎疗效肯定。
Objective To observe the clinical curative effect of combined treatment of Chinese and Western medicine on Helicobacter pylori (Hp) related erosive gastritis. Methods 110 patients with Hp-related erosive gastritis were randomly divided into two groups. The two groups were given anti-Hp triple therapy (omeprazole 20mg, amoxicillin 1.0g, furazolidone 0.1g, 2 times a day orally) for 1 week. One week after treatment, 56 cases were given omeprazole 20mg / time twice daily, Dragon’s Blood Capsule 4 capsules / time 3 times a day, orally for half an hour before meals; Teprenone 50mg / time 3 times a day, Oral for half an hour after a meal Control group of 54 cases to omeprazole 20mg / time 2 times a day, half an hour before meals orally. Both groups were treated for 4 weeks as a course of treatment. The therapeutic effect was judged after 1 month of withdrawal. Endoscopy was performed to examine the histopathological examination of Hp and gastric mucosa. The scores of gastroscope curative effect and gastric mucosal inflammation were also scored. Results The score of each symptom before and after treatment in both groups decreased, with statistical significance (P <0.01), and the improvement of stomach pain, fullness, heartburn, pantothenic acid and belching in the treatment group was better than that in the control group P <0.05). The total effective rate of the treatment group and the control group were 85.8% and 55.6%, respectively. There was significant difference between the two groups (P <0.01). Hp eradication rate was 83.9% in the treatment group and 77.8% in the control group. There was no significant difference between the two groups (P> 0.05). Conclusion Omeprazole, teprenone, dragon’s blood dextrose combined with anti-Hp triple therapy for Hp-related erosive erosive gastritis positive effect.
