目的建立新疆地区新生儿先天性甲状腺功能减低症(congenital hypothyroidism,CH)促甲状腺素(TSH)筛查实验切值。方法对出生72h经充分哺乳后的新生儿采集足跟血滤纸标本,采用时间分辨荧光免疫法(DELFIA),使用PerkinElmer试剂盒检测滤纸干血斑中促甲状腺素(TSH)浓度。采用百分位数法确定切值,并应用SPSS13.0软件分析筛查切值。结果新生儿筛查TSH水平呈偏态分布,正常新生儿99%百分位数值为7.77μU/ml,确诊38例阳性患儿,CH发生率1/1114,尚无漏诊报道。结论新疆地区新生儿中CH的发生率高于全国平均水平,筛查切值8.0μU/ml较适合新疆人群,新生儿TSH筛查切值的确立为新疆地区今后的新生儿CH筛查提供了依据。
Objective To establish a TSH screening test for congenital hypothyroidism (CH) in newborns in Xinjiang. Methods heel blood filter paper samples were collected from neonates after full lactation 72h after birth. The concentration of thyrotropin (TSH) in dried blood spots of filter paper was detected by time-resolved fluorescence immunoassay (DELFIA) using PerkinElmer kit. The cut-off value was determined using the percentile method, and the cut-off value was analyzed using SPSS 13.0 software. Results The level of TSH in neonatal screening was skewed. The 99th percentile of normal newborns was 7.77μU / ml. The positive rate of CH was 38% in children with CH, and the incidence of CH was 1/1114. Conclusions The incidence of CH in newborns in Xinjiang is higher than the national average. The screening cut-off value of 8.0 μU / ml is more suitable for Xinjiang people. The establishment of TSH screening value in newborns provides the newborn CH screening in Xinjiang in accordance with.
