目的:①研究额叶肿瘤患者的执行功能是否存在损害。②比较左额叶肿瘤患者与右额叶肿瘤患者的执行功能损害差异。方法:27名左额叶肿瘤患者、27名右额叶肿瘤患者、30名正常对照者,选用威斯康辛卡片分类测验(WCST)和连线测验进行执行功能评估。结果:①在WCST的完成分类数、完成第一次分类所需应答数、概念化水平百分数、持续性应答数以及持续性错误百分数5个指标上,左额叶组均显著差于右额叶组(P<0.05)以及正常组(P<0.05);而右额叶组的5个指标也显著的差于正常组(P<0.05)。②连线测验A的完成时间,连线测验B的完成时间与错误数上,左额叶组均显著差于右额叶组(P<0.05)和正常组(P<0.05);右额叶组在连线A完成时间与正常组没有显著差异(P=0.525),但在连线B完成时间和错误数上也显著差于正常组。结论:额叶肿瘤严重损害了成年人的执行功能,左额叶肿瘤对执行功能的损害程度更严重。
Objectives: ① To study whether the executive function of patients with frontal lobes is damaged. ② Comparison of executive function impairment between patients with left frontal lobes and right frontal lobes. Methods: Twenty-seven patients with left frontal lobectomy, 27 patients with right frontal lobectomy, and 30 normal controls were enrolled in this study. The executive function was assessed using the Wisconsin Card Sorting Test (WCST) and the connection test. Results: ①The left frontal lobe group was significantly worse than the right frontal lobe group in terms of the number of completed WCST, the number of responses required to complete the first classification, the level of conceptualization, the number of persistent responses and the percentage of persistent errors (P <0.05) and normal group (P <0.05), while the five indexes of right frontal lobe group were also significantly worse than those of normal group (P <0.05). ② The completion time of connection test A and the completion time and error number of connection test B were significantly lower in the left frontal lobe group than in the right frontal lobe group and the normal group (P <0.05) The group did not differ significantly from the normal group at connection A completion (P = 0.525), but also significantly worse than the normal group at connection B completion time and error number. CONCLUSIONS: Frontal lobe tumors severely impair the executive function of adults, and left-frontal tumors are more detrimental to executive function.
