目的:探讨血清胱抑素 C(Cys-C)、β2-微球蛋白(β2-MG)和同型半胱氨酸(Hcy)联检水平在糖尿病肾病(DN)早期诊断中的应用价值。方法:检测124例2型糖尿病(DM)患者和62例健康体检者血清 Cys-C、β2-MG、Hcy、肌酐(Creat)、尿素(Urea)的水平,并对结果进行分析。结果:单纯 DM 组Cys-C、β2-MG 和 Hcy 水平明显高于对照组(p<0.05),早期 DN 组 Cys-C、β2-MG 和 Hcy 水平显著高于单纯 DM 组(p<0.01);Creat 和 Urea 水平高于对照组和单纯 DM 组,但差异无统计学意义(p>0.05),血清 Cys-C、β2-MG、Hcy 分别与 Creat、Urea 呈正相关。单纯 DM 组中三项联检阳性检出率高于单项指标阳性检出率(p>0.05),早期 DN 组中三项单项阳性检出率明显高于单纯 DM 组阳性检出率(p<0.05),三项联检阳性检出率高于单项指标阳性检出率(p<0.05)。结论:血清 Cys-C、β2-MG 和 Hcy 是 DN 早期肾功能损伤的重要监测指标,联检有利于提高早期 DN 的检出率,在 DN 的早期诊断、病情评估及监测方面为临床提供实验室依据。“,”Objective:To discuss the combined testing level of Cystatin-C (Cys-C),β2-microglobulin (β2-MG) and Homocysteine(Hcy) the diabetic nephropathy(DN) application of in the early diagnosis of diabetic value. Methods: Serum levels of Cys-C, Hcy, β2-MG, creatinine (Creat) and urea (Urea) were detected in 2 patients with type 124 diabetes mel itus (DM) and 62 healthy subjects, and the results were analyzed by. Results: Pure DM group Cys-C,β2-MG ,Hcy level were significantly higher than that of control group (P0.05), early DN group in a single positive rate was significantly higher than that of pure DM group the positive rate (P<0.05), three joint inspection of positive testing rate is higher than the single index of positive testing rate (P<0.05). Conclusion: Serum Cys-C,β2-MG and Hcy were an important index for the monitoring of early renal injury in DN, joint detection is helpful to improve the early detection rate of DN in the early diagnosis of DN, disease monitoring and evaluation to provide laboratory basis for clinical.
