目的:探讨胸腔积液中脑钠肽(BNP)和血清腺苷脱氨酶(ADA)的水平对鉴别诊断结核性、恶性胸腔积液的临床价值。方法:选择结核性胸腔积液患者26例、恶性胸腔积液患者30例,分别取患者胸腔积液5 ml送检,采用ELISA法测定胸腔积液中BNP水平,用比色分析法检测胸腔积液ADA含量,并对测定结果进行统计学分析。结果:恶性胸腔积液中BNP水平明显高于结核性组,ADA水平明显低于结核性组,两组比较差异有统计学意义(P<0.01)。BNP与ADA在同一性质胸腔积液中的检测结果呈负相关性。结论:胸腔积液中BNP、ADA水平的测定有助于结核性及恶性胸腔积液的鉴别诊断,两者联合进行诊断的意义有待于进一步研究。
Objective: To investigate the clinical value of pleural effusion of brain natriuretic peptide (BNP) and serum adenosine deaminase (ADA) in the differential diagnosis of tuberculous and malignant pleural effusion. Methods: Twenty-six patients with tuberculous pleural effusion and 30 patients with malignant pleural effusion were selected. The patients were treated with 5 ml pleural effusion. The BNP level in pleural effusion was measured by ELISA. The pleural effusion volume Liquid ADA content, and the results of the statistical analysis. Results: The level of BNP in malignant pleural effusion was significantly higher than that in tuberculous group, and the level of ADA was significantly lower than that in tuberculous group. There was significant difference between the two groups (P <0.01). BNP and ADA in the same nature of pleural effusion test results showed a negative correlation. Conclusion: The determination of BNP and ADA levels in pleural effusions is helpful to the differential diagnosis of tuberculous and malignant pleural effusion. The significance of the combination diagnosis of the two remains to be further studied.
