目的检测妊娠期高血压疾病患者胎盘中乙酰肝素酶(HPA)、基质金属蛋白酶-9(MMP-9)的表达,探讨其在妊娠期高血压疾病发病中的作用。方法采用免疫组织化方法检测65例妊娠期高血压疾病患者(妊娠期高血压疾病组)及20例正常妊娠妇女(对照组)胎盘组织中HPA、MMP-9的表达。结果 1)妊娠期高血压疾病组与对照组比较胎盘组织中乙酰肝素酶表达明显降低(P<0.01)。轻、重度子痫前期患者胎盘组织中乙酰肝素酶表达强度均明显低于对照组(P<0.05,P<0.01)。重度子痫前期组胎盘组织中乙酰肝素酶表达明显低于轻度子痫前期组(P<0.05)。2)妊娠期高血压病组与对照组比较胎盘组织中MMP-9表达明显降低(P<0.05)。妊娠期高血压和轻、重度子痫前期患者胎盘组织中MMP-9表达逐渐减少,各组间差异有统计学意义(P<0.05)。3)妊娠期高血压病组胎盘组织中HPA与MMP-9表达呈正相关(r=0.388,P<0.01)结论孕妇胎盘组织中HPA、MMP-9表达水平的变化与妊娠期高血压疾病的发病及病情发展有关。
Objective To detect the expression of heparanase (HPA) and matrix metalloproteinase-9 (MMP-9) in the placenta of patients with gestational hypertension and to explore their roles in the pathogenesis of hypertensive disorder complicating pregnancy. Methods Immunohistochemistry was used to detect the expression of HPA and MMP-9 in placenta of 65 patients with gestational hypertension (gestational hypertension group) and 20 normal pregnant women (control group). Results 1) Compared with the control group, the expression of heparanase in placental tissue of patients with gestational hypertension was significantly decreased (P <0.01). The expression of heparanase in placenta of mild and severe preeclampsia patients was significantly lower than that of the control group (P <0.05, P <0.01). The expression of heparanase in placenta of severe preeclampsia group was significantly lower than that in mild preeclampsia group (P <0.05). 2) Compared with the control group, the expression of MMP-9 in placental tissue of hypertensive patients during pregnancy was significantly lower (P <0.05). The expression of MMP-9 in gestational hypertension and mild and severe preeclampsia patients decreased gradually, with significant difference between the groups (P <0.05). 3) There was a positive correlation between the expression of HPA and MMP-9 in placenta of patients with gestational hypertension (r = 0.388, P <0.01) .Conclusion The changes of HPA and MMP-9 in placenta of pregnant women are associated with the incidence of hypertensive disorder complicating pregnancy And the development of the disease.
