目的:探究对糖尿病患者开展跟踪管理的效果。方法:研究对象为2015年1~6月浙江省舟山市人民医院收治的143例糖尿病患者,患者出院后均采用跟踪管理进行指导,内容主要包括集中授课和个体随访两种方式,为患者建立健康档案,针对患者的病情程度设计指导计划,采用问卷调查了解患者在管理前后自我监测、合理饮食、规律运动、遵医嘱服药、按时检查的依从性及护理满意度。结果:在实施跟踪管理后,患者的自我监测、合理饮食、规律运动、遵医嘱服药、按时检查依从率,均显著高于管理前(P<0.05)。管理后患者的护理满意度为100.0%,显著高于管理前的94.4%(P<0.05)。结论:跟踪管理能够有效改善糖尿病患者的认知状况,提高患者的治疗依从性及护理满意度。
Objective: To explore the effect of tracking management of patients with diabetes. Methods: The study was conducted on 143 diabetic patients admitted to Zhoushan People’s Hospital of Zhejiang Province from January to June in 2015. All patients were followed up for follow-up guidance after discharge. The main contents included centralized teaching and individual follow-up, which established health for patients File, the patient’s condition for the design of the guiding plan, the use of questionnaires to understand patients before and after self-monitoring, reasonable diet, regular exercise, prescribed medication, on-time examination of compliance and nursing satisfaction. Results: After the implementation of follow-up management, patients’ self-monitoring, reasonable diet, regular exercise, prescribed medication and on-time examination of compliance rate were significantly higher than those before management (P <0.05). Patient satisfaction after management was 100.0%, significantly higher than 94.4% before management (P <0.05). Conclusion: The follow-up management can effectively improve the cognitive status of patients with diabetes and improve the patient’s compliance with treatment and nursing satisfaction.
