目的 探讨X线、CT、MRI在军人早期疲劳性骨折中的影像诊断价值,分析不同影像学检查方法 在疲劳性骨折早期诊断中的价值.方法 收集2012年3月-2017年11月中国人民武装警察部队北京市总队第三医院经临床随访或手术病理证实的40例军人疲劳性骨折患者,对其X线、CT和MRI影像学资料进行回顾性分析.结果 40例初诊患者中,8例X线、CT仅显示轻度骨膜反应,髓腔内密度正常时,MRI已明确显示骨髓和周围软组织水肿、骨膜增生改变;11例X线、CT已显示骨膜反应,髓腔内局部密度有改变时,MRI能更清晰显示骨髓和周围软组织水肿、骨膜增生改变的程度和范围;16例X线、CT未显示低密度骨折线时,MRI已明确显示典型的双低信号的骨折线;MRI能更清晰显示另5例跖骨陈旧疲劳性骨折病变及周围改变.结论 MRI比X线、CT在早期诊断军人疲劳性骨折中的价值更大,当怀疑疲劳性骨折,X线、CT检查结果 为阴性时,也要及早进行MRI检查.“,”Objective To investigate the diagnostic value of X-ray,CT and MRI in early fatigue fractures in soldiers, and to analyze which imaging methods are more valuable in early diagnosis of fatigue fractures. Methods Forty patients with fatigue fractures confirmed by clinical follow-up or pathology from March 2012 to November 2017 in our hospital were collected and their X-ray, CT, and MRI imaging data were analyzed retrospectively.Results Among the 40 newly diagnosed patients, 8 cases with X-ray and CT showed only mild periosteal reaction. When the intramedullary density was normal, MRI had clearly demonstrated bone marrow and surrounding soft tissue edema and periosteal hyperplasia. 11 cases of X-ray and CT showed periosteum. When the local density in the intramedullary cavity changes, MRI can more clearly show the extent and extent of bone marrow and surrounding soft tissue edema and periosteal hyperplasia. When 16 cases of X-ray and CT showed no low-density fracture lines, MRI has clearly shown typical Double low signal fracture line; MRI can more clearly show the other 5 old tibial fatigue fracture lesions and peripheral changes.Conclusions MRI is more valuable than X-ray and CT in early diagnosis of fatigue fractures in military. When fatigue fractures are suspected and X-ray and CT findings are negative, MRI should be performed early.
