马凡氏综合征患者脊柱侧凸的矫治

目的:探讨马凡氏综合征(Marfansyndrome,MFS)患者脊柱侧凸的手术疗效。方法:对1997年9月至2002年1月使用脊柱三维矫形内固定系统后路手术治疗的8例MFS患者脊柱侧凸的矫正度、脊柱平衡及并发症等进行分析。结果:8例患者随访12~48个月,平均24个月。6例KingⅡ型患者胸弯及腰弯的矫正率分别为47%、57%,随访矫正率丢失分别为21%和16%;1例KingⅠ型患者胸弯及腰弯矫正率分别为25%及42%,随访矫正率丢失分别为21%和15%;1例KingⅢ型患者胸弯及腰弯的矫正率分别为58%、29%,随访矫正率丢失分别为9%和18%。所有患者躯干平衡得到改善,无神经并发症发生。2例出现交界性腰椎后凸,1例切口延迟愈合。结论:使用脊柱三维矫形内固定系统后路手术治疗马凡氏综合征患者脊柱侧凸可以获得满意疗效。 Objective: To investigate the surgical treatment of scoliosis in patients with Marfan syndrome (MFS). Methods: The correction, spinal balance and complications of 8 patients with MFS treated with posterior approach of the spine three-dimensional orthopedic fixation system from September 1997 to January 2002 were analyzed. Results: Eight patients were followed up for 12 ~ 48 months with an average of 24 months. The correction rate of thoracolumbar and lumbar curve was 47% and 57% respectively in 6 cases of King Ⅱ type, and the follow-up correction rate was 21% and 16% respectively. The correction rate of thoracolumbar and lumbar curve in 1 case of KingⅠ was 25% and 42%, and the follow-up correction rate was 21% and 15% respectively. The correction rate of chest type and lumbar curve was 58% and 29% respectively in one case of KingⅢ type, and the follow-up correction rate was 9% and 18% respectively. Torso balance improved in all patients with no neurological complications. 2 cases of borderline lumbar kyphosis, delayed wound healing in 1 case. Conclusion: The posterior approach to the treatment of patients with Marfan syndrome scoliosis using the three-dimensional spinal orthopedic fixation system can achieve satisfactory results.
