目的探讨硝黄汤联合匀浆饮食鼻饲对神经外科监护室患者营养状态及肠道功能的影响。方法选取2015年3月—2016年3月在该院神经外科进行手术治疗后入住监护室的重症患者141例,根据术后鼻饲方式分为对照组(n=71)和观察组(n=70)。对照组给予常规鼻饲匀浆饮食肠内营养,观察组同时给予中药硝黄汤治疗。二组均治疗7d。检测并比较二组患者术后营养状态及肠道功能相关指标;统计二组临床疗效。结果治疗7d后二组TP、PA、TF、RBP等血清营养指标均明显上升,且观察组高于对照组(P<0.05或P<0.01)。治疗3~7d后二组每分钟肠鸣音次数较治疗前升高,且观察组高于对照组(P<0.01);治疗3~7d后对照组DAO、L/M呈升高趋势,观察组均呈下降趋势,且均低于对照组(P<0.05或P<0.01)。治疗7d后观察组肠道菌群改善情况明显优于对照组(P<0.05或P<0.01)。治疗后观察组总有效率85.71%,明显高于对照组的54.93%(P<0.01)。结论神经外科监护室患者术后应用硝黄汤联合匀浆饮食鼻饲能够促进术后肠道功能恢复,改善患者营养状态,疗效确切,值得临床推广与应用。
Objective To investigate the effect of nourishing nourishing soup combined with nourishing nasal feeding on nutritional status and intestinal function in neurosurgery intensive care unit. Methods A total of 141 critically ill patients admitted to the intensive care unit from March 2015 to March 2016 undergoing neurosurgery in the hospital were divided into control group (n = 71) and observation group (n = 70) according to postoperative nasal feeding method ). The control group was given conventional nasal feeding and enteral nutrition, and the observation group was treated with Chinese traditional medicine nitrate yellow soup. The two groups were treated for 7 days. The nutritional status and intestinal function related indexes of two groups were detected and compared. The clinical efficacy of the two groups were statistically analyzed. Results Serum nutrition indexes such as TP, PA, TF, RBP in the two groups were significantly increased after 7 days of treatment, and the observation group was higher than the control group (P <0.05 or P <0.01). After 3 ~ 7 days of treatment, the number of bowel sounds per minute in two groups increased compared with that before treatment, and the observation group was higher than that of the control group (P <0.01); DAO and L / M of the control group increased after 3-7 days of treatment, Group showed a downward trend, and were lower than the control group (P <0.05 or P <0.01). After 7 days of treatment, the improvement of intestinal microflora in the observation group was significantly better than that of the control group (P <0.05 or P <0.01). After treatment, the total effective rate in observation group was 85.71%, which was significantly higher than that in control group (54.93%, P <0.01). Conclusion The patients with neurosurgery nursing room after application of rhubarb soup combined with homogenate diet nasal feeding can promote postoperative intestinal function recovery, improve the nutritional status of patients, the exact effect, worthy of clinical promotion and application.
