目的:开展Rh血型抗原检测技术,利用实验室血型自动分析软件系统和输血信息管理软件系统等计算机信息识别技术,探讨Rh血型5种抗原检测在精准化输血中应用的可行性。方法:利用实验室血型全自动分析系统,对献血员红细胞标本和受血者标本都进行ABO血型和Rh 5种血型抗原(D、C、c、E和e)检测,将检测结果上传至输血信息管理系统,利用计算机识别技术为受血者自动匹配ABO血型和Rh 5种血型抗原相合的血袋,用凝聚胺法及抗人球蛋白卡式法进行交叉配血试验,实现临床精准化输血。结果:实施输血精准化方案后,90%以上的受血者可以实现ABO血型和Rh 5种血型抗原完全相合的输血,未见输血不良反应发生;与实施精准化输血方案前相比,多次输血或有输血史的患者血型不规则抗体阳性明显降低,且在不规则抗体阳性情况下疑难配血成功率大大提高。结论:开展Rh血型抗原检测技术实现临床精准化输血,既可以避免Rh系统抗体的产生,同时也减少了输血不良反应的发生;提升了解决疑难配血的能力,提高了输血治疗的安全性。
OBJECTIVE: To carry out Rh blood group antigens detection technology and to use computer information recognition technology such as laboratory blood type automatic analysis software system and blood transfusion information management software system to explore the feasibility of using Rh blood group 5 antigen detection in accurate blood transfusion. Methods: The blood samples of red blood cells and recipients of blood donors were detected by ABO blood group and Rh 5 blood group antigens (D, C, c, E and e) by laboratory automatic blood analysis system. The test results were uploaded to blood transfusion Information management system, the use of computer-based recognition technology for blood recipients automatically match ABO blood type and Rh 5 blood group antigen matched blood bag, with polybrene method and anti-human globulin card cross-matching blood test method to achieve clinical accurate blood transfusion . Results: After the blood transfusion accurate program was implemented, more than 90% of recipients could achieve complete transfusion of ABO blood type and Rh 5 blood group antigens without any adverse transfusion reactions. Compared with the pre-transfusion program, In patients with blood transfusion or blood transfusion irregular blood group antibody was significantly lower, and in the case of irregular antibodies positive difficulty with blood distribution greatly increased. CONCLUSIONS: To carry out Rh blood group antigen detection technology to achieve clinical accurate blood transfusion can not only avoid the production of Rh system antibodies, but also reduce the incidence of transfusion adverse reactions; enhance the ability to solve difficult blood distribution and improve the safety of blood transfusion therapy.
