目的观察国产雷帕霉素洗脱支架(FirebirdTM支架)在冠心病合并糖尿病患者介入治疗中的疗效。方法2004年1月至2006年10月我院住院的合并糖尿病的冠心病患者95例,经常规冠状动脉造影检查,进行冠状动脉介入治疗。其中52例植入国产雷帕霉素洗脱支架,43例植入普通裸支架。所有患者术前、术后均常规给予阿司匹林及氯吡格雷等药物治疗,术后进行随访。结果支架植入成功率为100%,随访9个月,国产雷帕霉素洗脱支架组复发心绞痛3例,心肌梗死1例,心源性死亡1例。普通裸支架组复发心绞痛11例,心肌梗死5例,心源性死亡4例。其中国产雷帕霉素洗脱支架组19例6～8个月复查冠状动脉造影,支架内再狭窄2例;普通裸支架组21例6～8个月复查冠状动脉造影,支架内再狭窄10例。两组比较有统计学差异。结论国产雷帕霉素洗脱支架对冠心病合并糖尿病患者的疗效确切,能减少支架内再狭窄的发生,降低不良心血管事件发生率。
Objective To observe the curative effect of domestic-made rapamycin-eluting stent (FirebirdTM stent) in interventional therapy of coronary heart disease complicated with diabetes mellitus. Methods From January 2004 to October 2006, 95 hospitalized diabetic patients with coronary heart disease in our hospital underwent routine coronary angiography and coronary intervention. Among them, 52 cases were implanted with domestic rapamycin-eluting stent and 43 cases with common bare stent. All patients were routinely given aspirin and clopidogrel preoperative and postoperative drugs, postoperative follow-up. Results The successful rate of stent implantation was 100%, followed up for 9 months. There were 3 cases of recurrent angina pectoris, 1 case of myocardial infarction and 1 case of cardiogenic death in domestic rapamycin - eluting stent group. Eleven patients had recurrent angina in general bare stent group, 5 in myocardial infarction, and 4 in cardiac death. Among them, 19 cases of domestic rapamycin-eluting stent group underwent coronary angiography in 6 to 8 months and 2 cases of in-stent restenosis; 21 cases of common bare stent group underwent coronary angiography in 6 to 8 months and in-stent restenosis 10 example. The two groups were statistically different. Conclusion The domestic rapamycin-eluting stent has definite effect on patients with coronary heart disease complicated with diabetes mellitus, can reduce the incidence of in-stent restenosis and reduce the incidence of adverse cardiovascular events.
