近年来国内外利用经颅多普勒诊断脑血管病变,这一无创性颅脑内血管病变检测手段,在临床大量的脑血管疾病的诊疗以及指导治疗方面应用较广泛。用TCD(经颅多普勒)诊断仪以及双功能彩色多普勒经颅检测仪对(1)椎,基底动脉及幕上动脉流速异常致供血不足。脑血管机能紊乱。颈性眩晕等引起的头晕。(2)动脉硬化,TCD表现血流速、声频、频谱形态及血管搏动指数的异常。(3)颅内动脉栓塞所致血管狭窄和动脉闭塞的TCD异常频谱及检测方法。(4)烟雾病的诊断,TCD是首选也是特意性检测的重要手段。并有其独特征象。(5)蛛网膜下腔出血所致动脉痉挛可致DINDS(迟发性缺血性神经系统综合症),是使患者致残与死亡的重要原因之一。TCD诊断动脉痉挛的意义在于及时发现,并采取积极有效治疗措施防止病情发展。(6)脑动静脉畸形TCD主要表现为盗血现象。大量本应供应正常脑区的血液向阻力低的动静脉畸形血管灌注,致正常脑区缺血。(7)脑死亡TCD频谱有其典型的血流异常表现。但目前仍处在研究探讨之中。(8)颈动脉海绵窦漏TCD可测到频谱波型率乱或波峰融合,以及边缘不清,呈毛刺状,有收缩期,吹风样杂音等异常频谱致变。
