目的探讨多层螺旋CT(MSCT)任意角度曲面重建(CPR)胆管成像技术在胆总管扩张病变诊断中的应用价值。方法 36例胆总管扩张患者,其中均采用8mm层厚、8mm层距、螺距0.9行腹部MSCT容积扫描获得原始数据,采用1mm层厚、1mm层距重建轴面源像(ASI)在工作站行扩张胆管曲面重建成像,经手术病理及随访证实,评估CPR对胆总管扩张病因的定位、定性诊断价值。结果 36例CPR胆道成像率达100%,均能完整显示扩张胆总管及梗阻部位,定位准确率达100%,定性诊断率达94.4%。结论 CPR可直观显示胆总管扩张程度、部位及梗阻端形态,对胆道梗阻及鉴别诊断有重要价值。
Objective To investigate the value of multislice spiral CT (MSCT) arbitrary angle reconstruction (CPR) in the diagnosis of common bile duct dilatation. Methods Thirty - six patients with common bile duct dilatation were enrolled in this study. The raw data were obtained by abdominal MSCT volume scanning with 8mm layer thickness, 8mm layer pitch and pitch of 0.9. Using a 1mm layer thickness and 1mm layer distance reconstructed axial source image (ASI) The reconstruction of the bile duct surface imaging confirmed by surgical pathology and follow-up to assess the cause of CPR dilatation of the common bile duct location, the value of qualitative diagnosis. Results 36 cases of CPR biliary imaging rate of 100%, can completely display the dilatation of the common bile duct and obstruction site, positioning accuracy of 100%, the qualitative diagnosis rate of 94.4%. Conclusion CPR can directly show the degree of dilatation of the common bile duct, location and shape of the obstructive end of bile duct obstruction and differential diagnosis of great value.
