目的观察和分析隐蔽性高血压(MHT)患者血浆同型半胱氨酸(Hcy)和颈股动脉脉搏波传导速度(cfPWV)的水平及其相互关系。方法本研究为横断面研究,纳入202名正常血压者为对照组,MHT患者226例为观察组;两组受试者根据血浆Hcy水平进一步分为高Hcy亚组(Hcy≥10μmol/L)或正常Hcy亚组(Hcy<10μmol/L)。检测各研究对象血浆Hcy水平和cfPWV的水平,比较两组间的差异,分析其相互关系。结果 MHT组血浆Hcy和cfPWV水平高于正常血压组。在MHT患者中,高Hcy亚组血浆Hcy和cfPWV水平高于正常Hcy亚组(均P<0.05);而在正常血压组人群中,高Hcy亚组与正常Hcy亚组cfPWV的差异无统计学意义(P>0.05)。偏相关分析显示,MHT患者血浆Hcy与cfPWV呈正相关(r=0.335,P<0.05)。调整混杂因素后,多元回归分析显示,在MHT患者中,血浆Hcy水平是cfPWV的影响因素(B=1.125,P<0.05)。结论 MHT患者血浆Hcy与cfPWV呈正相关,血浆Hcy水平是cfPWV的影响因素。
Objective To observe and analyze the plasma levels of homocysteine (Hcy) and carotid artery pulse wave velocity (cfPWV) in patients with latent hypertension (MHT) and their correlations. METHODS: This study was a cross-sectional study involving 202 normotensive patients as control group and 226 MHT patients as observation group. Subjects in both groups were further divided into high Hcy group (Hcy≥10μmol / L) or Normal Hcy subgroup (Hcy <10μmol / L). Plasma Hcy levels and cfPWV levels in each study were measured, and the differences between the two groups were compared to analyze the relationship between them. Results The plasma levels of Hcy and cfPWV in MHT group were significantly higher than those in normotensive group. In MHT patients, the levels of plasma Hcy and cfPWV in high Hcy subgroups were higher than those in normal Hcy subgroups (all P <0.05). However, no significant differences were found in the plasma levels of cfPWV between high Hcy subgroups and normal Hcy subgroups Significance (P> 0.05). Partial correlation analysis showed that there was a positive correlation between plasma Hcy and cfPWV in patients with MHT (r = 0.335, P <0.05). After adjusting for confounding factors, multivariate regression analysis showed that plasma Hcy level was the influencing factor for cfPWV in MHT patients (B = 1.125, P <0.05). Conclusions There is a positive correlation between plasma Hcy and cfPWV in MHT patients, and plasma Hcy level is the influencing factor of cfPWV.
