目的探究阴道镜Reid评分对筛查HIV阳性妇女宫颈癌的临床诊断价值。方法选取确诊的HIV阳性妇女284例作为研究对象,并将其随机分为观察组和对照组各142例。观察组患者进行阴道镜检查并结合Reid评分,对照组患者进行液基薄层细胞学检测,对于检测结果异常的患者进行多点活检病理检查。分析两组患者的筛查结果与病理检查的符合率,比较两种筛查方法的灵敏度、特异度和阳性率。结果观察组Reid评分与多点活检组织病理学检查结果符合率75.35%,对照组患者细胞学检测与组织病理学检查结果符合率40.85%,两组比较差异有统计学意义(P<0.01)。对照组的灵敏度稍高于观察组,差异无统计学意义(P>0.05);观察组筛查的特异度为85.23%,明显优于对照组的67.98%,差异有统计学意义(P<0.01)。观察组的阳性率远高于对照组(P<0.01)。结论对HIV阳性妇女患者进行阴道镜Reid评分检查效果好,能准确筛查宫颈癌病变,对宫颈癌的防治具有重要意义,值得进一步推广应用。
Objective To investigate the value of colposcopy Reid score in screening of cervical cancer in HIV-positive women. Methods Totally 284 confirmed HIV positive women were selected as research object and randomly divided into observation group (142 cases) and control group (142 cases). Patients in the observation group underwent colposcopy combined with Reid scores. The patients in the control group underwent liquid-based thin-layer cytology. Multiple biopsy pathological examination was performed on patients with abnormal results. Analysis of two groups of patients screening results and pathological examination of the compliance rate, the two screening methods compared the sensitivity, specificity and positive rate. Results The Reid score of the observation group was 75.35% with the biopsy histopathological examination results, and the coincidence rate of cytology and histopathological examination in the control group was 40.85%. There was significant difference between the two groups (P <0.01). The sensitivity of the control group was slightly higher than that of the observation group, with no significant difference (P> 0.05). The specificity of screening in the observation group was 85.23%, which was significantly better than that of the control group (67.98%), the difference was statistically significant (P <0.01) ). The positive rate of the observation group was much higher than that of the control group (P <0.01). Conclusions Colposcopy Reid scoring is effective for HIV-positive women and can accurately detect the cervical lesions, which is of great significance for the prevention and treatment of cervical cancer. It is worth further promotion and application.
