目的探讨脂肪细胞脂肪酸结合蛋白(FABP4)与糖尿病肾病早期的相关性。方法实验分为糖尿病肾病早期组46例和正常对照组40例,用ELISA方法检测FABP4,检测空腹血糖(FBP)、糖化血红蛋白(HbA1c)、胰岛素、血脂和肌酐(Scr),根据公式计算胰岛素敏感指数(ISI)和肾小球滤过率(eG-FR),用SPSS13.0统计软件进行统计分析。结果①糖尿病肾病组的体重指数(BMI)、FBG、三酰甘油(TG)、胆固醇(TC)、Scr和FABP4均较对照组增加(P<0.05),而ISI和eGFR则明显降低(P<0.05);②Spearman相关分析显示糖尿病肾病组血浆FABP4与BMI、FBG、HbA1c、TG、Scr正相关,r分别为0.16、0.17、0.22、0.20、0.31(P<0.05)。而与ISI和eGFR负相关r分别为-0.32(P<0.01)、-0.44(P<03.01)。结论糖尿病肾病早期患者的FABP4水平升高,与肾功能具有负相关性。
Objective To investigate the early association of FABP4 with diabetic nephropathy. Methods The experiment was divided into early diabetic nephropathy group (n = 46) and normal control group (n = 40). FABP4 was detected by ELISA and fasting blood glucose (FBP), HbA1c, insulin, blood lipid and creatinine were measured. Insulin sensitivity Index (ISI) and glomerular filtration rate (eG-FR), using SPSS13.0 statistical software for statistical analysis. Results ① The body mass index (BMI), FBG, triglyceride (TG), cholesterol, Scr and FABP4 in diabetic nephropathy group were significantly higher than those in control group (P <0.05), but ISI and eGFR were significantly lower (P < 0.05). (2) Spearman correlation analysis showed that there was a positive correlation between plasma FABP4 and BMI, FBG, HbA1c, TG and Scr in patients with diabetic nephropathy (r = 0.16,0.17,0.22,0.20,0.31, P <0.05). The negative correlations with ISI and eGFR were -0.32 (P <0.01) and -0.44 (P <03.01), respectively. Conclusion The level of FABP4 in early diabetic nephropathy patients is increased, which is negatively correlated with renal function.
