目的探讨飞行人员运动负荷试验心电图 T 波正常化的临床价值,为飞行人员健康鉴定提供参考。方法对72名无器质性心脏病和心肌缺血发作史,仅 T 波改变的男性飞行人员进行活动平板次极限量运动负荷试验,分析运动前、中、后12导联同步心电图的 T 波变化和临床体检资料。结果 48例(66.7%)随着运动负荷加大 T 波振幅逐渐增高;24例(33.3%)在低运动负荷时 T 波振幅反而较运动前低,持续3~5min 后随运动负荷加大再逐渐增高。T 波在运动达次极量心率时恢复正常1例(1.4%);运动终止后1min 内恢复正常6例(8.3%)、2min 正常62例(86.1%)、3min 正常3例(4.2%)。运动终止2min T 波又转低平1例(1.4%)、3min 转低平22例(30.6%)、4min 转低平39例(54.2%)、5min 转低平7例(9.7%),运动后6min T 波仍呈正常直立3例(4.2%)。结论单纯运动性 T 波正常化无明显临床价值,但应注意与病理性改变相鉴别。
Objective To investigate the clinical value of T wave normalization in pilots’ exercise stress test and to provide reference for the health evaluation of pilots. Methods Seventy-two male pilots who had no organic heart disease and history of myocardial ischemia and had only T-wave changes were subjected to the submaximal exercise load test. The T-wave of 12-lead synchronous ECG before, during and after exercise Change and clinical medical information. Results The amplitude of T wave increased gradually with the increase of exercise load in 48 cases (66.7%). The amplitude of T wave in 24 cases (33.3%) was lower than that before exercise at low exercise load and continued to increase with exercise load Increasingly. T wave returned to normal in 1 case (1.4%) when it reached maximal heart rate, 6 cases (8.3%) recovered within 1 minute after exercise, 62 cases (86.1%) were normal in 2 minutes and 3 cases (4.2% . In 2 minutes after the end of exercise, T wave turned low again in 1 case (1.4%), reduced to flat in 3 minutes (30.6%), decreased in 39 cases (54.2%) in 4 minutes, decreased in 7 cases (9.7% T wave after 6min was still normal erect in 3 cases (4.2%). Conclusion There is no obvious clinical value of normal exercise T wave alone, but should pay attention to differentiate from pathological changes.
