目的 探讨颅内钙化性表皮样囊肿的影像学表现及其与临床病理的关系。资料与方法回顾性分析6例颅内钙化性表皮样囊肿的CT和MRI表现,并与临床手术、病理对照研究。结果 6例颅内钙化性表皮样囊肿中额叶1例,颞叶2例,顶叶1例,后颅窝1例,四脑室1例。6例病灶周围均无水肿,病灶内钙化呈蛋壳状1例,斑点状4例,弯曲条带状1例。钙化位于病灶中心1例,位于边缘5例。增强扫描病灶边缘轻到中度环形强化4例,无明显强化2例。6例病理结果均为表皮样囊肿伴炎性肉芽肿及钙化。结论 颅内钙化性表皮样囊肿少见,病理多提示合并感染,增强扫描可以强化,仔细分析其影像学表现,有助于提高术前诊断准确率。
Objective To investigate the imaging findings of intracranial calcified epidermoid cyst and its relationship with clinical pathology. Materials and Methods Retrospective analysis of 6 cases of intracranial calcified epidermoid cyst CT and MRI findings, and clinical and pathological control study. Results In 6 cases of intracranial calcified epidermoid cysts, there were 1 frontal lobe, 2 temporal lobes, 1 parietal lobe, 1 posterior fossa, and 1 cerebral ventricle. There was no edema around the 6 lesions, 1 case of eggshell in calcification, 4 cases of spots and 1 case of curved bands. Calcification in the center of the lesion in 1 case, located in the edge of 5 cases. Forty patients with mild to moderate annular enhancement on the edge of enhanced scan showed no enhancement in 2 cases. Six cases of pathological findings were epidermoid cyst with inflammatory granuloma and calcification. Conclusions The intracranial calcified epidermoid cyst is rare, and many pathological findings suggest that the infection is complicated. The enhanced scan can be strengthened. Careful analysis of the imaging findings can help to improve the accuracy of preoperative diagnosis.
