目的分析黑白与彩色多普勒阴道超声不同检查时间对早孕胚胎绒毛超微结构的影响。方法选取人工流产的早孕者120例,随机分为两组,彩超组60例予阴道彩色多普勒超声检查,其中检查5 min者20例,10 min者18例,15 min者22例。黑白组60例予阴道黑白B超检查,其中检查5 min者19例,10 min者19例,15 min者22例。对比两组胚胎绒毛超微结构变异率及绒毛DNA单链断裂和双链断裂的情况。结果检查5 min和10 min,两组胚胎绒毛超微结构变异率差异无统计学意义(P>0.05);检查15 min,彩超组绒毛超微结构变异率高于黑白组(P<0.05)。检查5 min,两组DNA单链和双链断裂情况比较差异无统计学意义(P>0.05)。检查10 min及15 min,彩超组绒毛DNA单链、双链断裂均较黑白组增加,差异均有统计学意义(P<0.05)。彩超组:绒毛超微结构变异率,5 min组和10 min组无差异,15 min组高于10 min组。随着检查时间延长,DNA单链、双链断裂增加,5 min组<10 min组<15 min组。黑白组:检查15 min内,绒毛超微结构变异率无差异。15 min组DNA单链、双链断裂较10 min组增加(P<0.05)。结论阴道黑白超声检查15 min内,对胚胎绒毛超微结构影响较小。阴道彩色多普勒超声检查超过10 min,对胚胎绒毛超微结构影响会增加。超声检查选择应结合临床情况,并控制检查时间。
Objective To analyze the effect of different examination time of black and white and color Doppler vaginal ultrasound on the chorionic villi ultrastructure in early pregnancy. Methods 120 pregnant women with induced abortion were randomly divided into two groups. Color Doppler ultrasonography was performed in 60 cases. Among them, 20 cases were examined at 5 minutes, 18 cases at 10 minutes and 22 cases at 15 minutes. Black and white group of 60 cases to the vaginal B-ultrasound, including examination of 5 min in 19 cases, 10 min in 19 cases, 15 min in 22 cases. The mutation rates of the two groups of embryos villus ultrastructure and double-strand breaks of villi DNA were compared. Results After 5 min and 10 min, there was no significant difference in the variation of the ultrastructure between the two groups (P> 0.05). At 15 min, the variation of the ultrastructure of the chorionic villi in the ultrasound group was higher than that in the black group (P <0.05). Check 5 min, two groups of DNA single-strand and double-strand breaks were no significant difference (P> 0.05). At 10 min and 15 min, the single-stranded DNA and double-strand breaks of DNA in the DNA of the hyperchromic group were significantly higher than those in the black group (P <0.05). Color ultra-group: The variation rate of the ultrastructure of villus, 5 min group and 10 min group no difference, 15 min group was higher than 10 min group. With the extension of examination time, DNA single-stranded and double-strand breaks increased, while those in 5 min group <10 min group <15 min group. Black-and-white group: within 15 min of examination, there was no difference in the variation rate of the ultrastructure of villus. The DNA single-strand and double-strand breaks in 15 min group increased more than 10 min group (P <0.05). Conclusions Within 15 min of vaginal ultrasound examination, the ultrastructure of the embryo villus is less affected. Vaginal color Doppler ultrasound examination of more than 10 min, the impact on the ultrastructure of embryonic villi will increase. Ultrasound examination should be combined with the clinical situation, and control the examination time.
