目的观察缺氧缺血性脑病(hypoxic-ischemic encephalopathy,HIE)新生儿振幅整合脑电图(amplitude integrated electroencephalography,aEEG)的变化,分别研究aEEG分类与HIE临床分度、HIE头颅CT分度的关系,探讨aEEG在HIE诊断中的价值。方法选择48例有窒息史的HIE新生儿为研究对象。根据临床分度分成轻、中、重度HIE组,分别为20例、16例、12例;根据头颅CT分度分成轻、中、重度HIE组,分别为18、18、12例。按aEEG分类标准将研究对象分为正常、轻度异常和重度异常3种。观察48例HIE新生儿aEEG的特点。结果临床轻、中、重度HIE患儿aEEG异常率分别为13.3%、86.7%、100%,aEEG异常程度与HIE临床分度密切相关(r=0.853,P<0.000 5);头颅CT轻、中、重度HIE患儿aEEG异常率分别为10.5%、82.4%、100.0%,aEEG异常程度与HIE头颅CT分度密切相关(r=0.825,P<0.000 5)。结论 aEEG异常程度与HIE临床分度、HIE头颅CT分度均密切正相关,在早期诊断HIE、判断HIE病情程度及预测预后方面有很大的应用价值。
Objective To observe the changes of amplitude integrated electroencephalography (aEEG) in neonates with hypoxic-ischemic encephalopathy (HIE), and to study the relationship between aEEG classification and HIE cephalometric classification , To explore the value of aEEG in the diagnosis of HIE. Methods Forty-eight HIE neonates with asphyxia history were selected as the study subjects. The patients were divided into mild, moderate and severe HIE group according to clinical grade, which were 20 cases, 16 cases and 12 cases respectively. According to the classification of head CT, they were divided into mild, moderate and severe HIE group, 18,18,12 cases respectively. According to aEEG classification criteria, the subjects were divided into three groups: normal, mild and severe. 48 cases of HIE neonatal aEEG characteristics. Results The abnormal rates of aEEG in children with mild, moderate and severe HIE were 13.3%, 86.7% and 100% respectively. The abnormalities of aEEG were closely related to the clinical grade of HIE (r = 0.853, P <0.0005) . The abnormal rates of aEEG in children with severe HIE were 10.5%, 82.4% and 100.0%, respectively. The abnormality of aEEG was closely related to the classification of HIE head CT (r = 0.825, P <0.0005). Conclusion The abnormality of aEEG is closely related to the clinical classification of HIE and the index of CT of HIE. It is of great value in the early diagnosis of HIE, the severity of HIE and the prediction of prognosis.
