目的:探讨检测脂肪肝患者血清瘦素水平的临床意义,研究血清瘦素水平与脂肪肝患者病情轻重程度之间的关系。方法:对2009年11月~2012年10月期间我院收治的100例脂肪肝患者的临床资料进行回顾性研究。我们根据这100例患者进行肝脏B超检查的结果,将他们分为重度脂肪肝组和轻度脂肪肝组,每组各有50例患者,同时另选20名健康人作为对照组。检测这三组人员的血清瘦素、总胆固醇、高密度脂蛋白胆固醇、谷丙转氨酶、谷草转氨酶和体重指数等指标。结果:重度脂肪肝组患者的血清瘦素、总胆固醇、体质量指数、谷丙转氨酶和谷草转氨酶的水平均高于轻度脂肪肝组患者,而且重度脂肪肝组患者高密度脂蛋白的水平低于轻度脂肪肝组患者,二者相比差异具有显著性(p<0.05)。轻度脂肪肝组患者的血清瘦素、总胆固醇、体质量指数、谷丙转氨酶和谷草转氨酶的水平均高于对照组的人员,而且轻度脂肪肝组患者高密度脂蛋白的水平低于对照组的人员,二者相比差异具有显著性(p<0.05)。结论:脂肪肝患者的血清瘦素水平与其病情的轻重程度成正比。检测血清瘦素水平可作为诊断脂肪肝患者病情轻重程度的独立方法,值得在临床上推广使用。
Objective: To investigate the clinical significance of serum leptin levels in patients with fatty liver and to study the relationship between serum leptin level and the severity of patients with fatty liver. Methods: The clinical data of 100 patients with fatty liver treated in our hospital from November 2009 to October 2012 were retrospectively studied. Based on the results of liver ultrasonography in 100 patients, we divided them into severe fatty liver group and mild fatty liver group, 50 patients in each group and 20 healthy individuals as control group. Serum leptin, total cholesterol, high density lipoprotein cholesterol, alanine aminotransferase, aspartate aminotransferase and body mass index were detected in these three groups. Results: The levels of serum leptin, total cholesterol, body mass index, alanine aminotransferase and aspartate aminotransferase in patients with severe fatty liver were higher than those in patients with mild fatty liver, and patients with severe fatty liver had lower levels of high density lipoprotein In mild fatty liver group, the difference was significant (p <0.05). The levels of serum leptin, total cholesterol, body mass index, alanine aminotransferase and aspartate aminotransferase in patients with mild fatty liver were higher than those in control group, and the levels of high density lipoprotein in patients with mild fatty liver were lower than those in control The difference between the two groups was significant (p <0.05). Conclusions: Serum leptin levels in patients with fatty liver are directly proportional to the severity of the disease. Detection of serum leptin levels can be used as an independent method of diagnosis of fatty liver disease severity, it is worth to promote the clinical use.
