目的了解伊犁州经非婚异性性途径感染艾滋病病毒(HIV)者的感染情况,以及隐瞒真实感染途径的原因,为当地开展相关流行病学调查工作提供参考依据。方法在当地2014年1-6月抗病毒治疗数据库中,按系统随机抽样原则抽取60名报告为非婚异性性途径感染的HIV感染者,进行一对一、面对面定性访谈,以信息饱和为终止标准,并对感染者的抗病毒治疗管理医生就其感染途径等相关问题进行复核访谈。结果 60名访谈对象,年龄为19~66岁,维吾尔族占78.3%,无业和个体占41.7%;多异性性行为是非婚异性性途径感染HIV的主要原因;与阳性配偶无保护性行为是婚内异性性传播的主要原因;经济利益驱动导致低年龄男性提供男男性行为服务是感染HIV的原因;因访谈对象对HIV传播途径及预防的有效措施不了解而延误诊断时间,以至无法准确判断感染途径是感染途径为不详的原因。访谈对象中,隐瞒真实感染途径的原因主要有:因婚内外多异性性伴而无法准确判断感染途径、社会文化规范对男男性行为者的挤压、害怕因吸毒被强制监管和受到家庭朋友歧视等。结论当地报告因非婚异性性途径感染HIV的人群,其真实感染的原因有多种,应针对不同感染途径人群开展相应有效的干预措施。
Objective To understand the infection status of HIV infected by non-same sex in Yili Prefecture and the reason of concealing the true path of infection and to provide a reference for the related epidemiological investigation in Yili Prefecture. Methods Sixty-six HIV-infected individuals reported as non-marital heterosexuality were randomly selected according to the principle of systematic random sampling in the local anti-virus treatment database from January to June 2014. Among the interviewees, the main reasons for concealing the true route of infection were: the inability to accurately judge the route of infection due to heterosexual partners inside and outside the family, the social and cultural norms squeezed the men who have sex with men, the fear of forced supervision due to drug abuse and the discrimination of family and friends Wait. Conclusion There are many reasons for local infection of HIV-infected people in non-marital heterosexual reports. Corresponding and effective interventions should be given to people with different routes of infection.
