本文测定了84例癌性与结核性胸水患者血清和胸水纤维连接蛋白(Fn)、脂质结合唾液酸(LSA)和腺苷脱氨酶(ADA)等含量变化。癌性胸水组血清LSA、胸水LSA和Fn含量测定值均显著高于结核性胸水组(P<0.01)。胸水Fn(≥0.05g/L)对癌性胸水诊断灵敏度为84%、特异度为76%,准确度达80%。血清LSA(≥0.15g/L)的准确度为86%。而胸水ADA(≥45u/g)对结核性胸膜炎诊断的灵敏度为91%,特异度为87%。表明LSA和Fn是较灵敏而特异的肿瘤标志物,对癌性胸水具有较高的诊断价值。
In this study, serum and pleural fibronectin (Fn), lipid-associated sialic acid (LSA), and adenosine deaminase (ADA) levels were measured in 84 patients with cancerous and tuberculous pleural effusions. The levels of serum LSA and LSA and Fn in the pleural effusion group were significantly higher than those in the tuberculous pleural effusion group (P<0.01). The diagnostic sensitivity of pleural fluid Fn (≥0.05g/L) to cancerous pleural effusion was 84%, specificity was 76%, and accuracy was 80%. The accuracy of serum LSA (≥0.15 g/L) was 86%. The pleural fluid ADA (≥45u/g) has a sensitivity of 91% and a specificity of 87% for the diagnosis of tuberculous pleurisy. It is shown that LSA and Fn are sensitive and specific tumor markers and have a high diagnostic value for cancerous pleural effusion.
