目的分析研究护士分层级管理模式对提高产科护理质量的作用。方法 2016年3月起本科使用护士分层级管理模式来管理护士,选取本科2015年3~12月阴道分娩242例产妇、15名护士及2016年3~12月阴道分娩230例产妇、15名护士作为研究对象,观察实施护士分层管理模式前后的护理情况。结果实施护士分层级管理后,产妇尿潴留、乳房肿胀、留置针非计划拔管发生率分别为2.6%、0.9%、5.7%,均明显低于实施前的10.7%、7.0%、18.2%,护理满意度89.1%明显高于实施前62.0%,差异均具有统计学意义(P<0.05)。实施护士分层级管理前共发生12次护理差错,而实施后无一次护理差错发生。实施护士分层级管理后产妇对护理质量各项评分均明显优于实施前(P<0.05)。实施护士分层级管理后护士对产妇情况掌握水平及护士操作技能评分均明显高于实施前(P<0.05)。结论实施护士分层级管理模式能够明显提高产科护理质量,提高孕产妇护理满意度,提高护理人员责任感和工作热情,值得在医院实施。
Objective To analyze and study the role of nursing management in different levels to improve the quality of obstetric care. Methods From March 2016, nurses were employed to manage nurses by using nurses’ hierarchical management mode. From March to December 2015, 242 maternal births, 15 nurses and 230 vaginal deliveries from March 2016 to December 2016 were selected. Fifteen Nurses as the object of study to observe the nursing situation before and after the implementation of the nurse management model. Results The incidence of maternal urinary retention, breast swelling and indwelling needle unplanned extubation were 2.6%, 0.9% and 5.7% respectively after being nurses administered at different levels, which were significantly lower than those of 10.7%, 7.0% and 18.2% , 89.1% of nursing satisfaction was significantly higher than 62.0% before implementation, the differences were statistically significant (P <0.05). There were altogether 12 nursing mistakes before implementing nursing management at different levels, but no nursing mistakes occurred after implementation. After the nurses were administered at different levels, the scores of nursing quality were significantly better than those before the implementation (P <0.05). Nurses ’mastery of maternal status and nurses’ skills of operation were significantly higher than those before implementation (P <0.05). Conclusion The implementation of nurses’ hierarchical management mode can obviously improve the quality of obstetrics and nursing care, raise the satisfaction of maternity nursing and enhance the sense of responsibility and enthusiasm of nurses, which is worth to be implemented in the hospital.
