目的估算山西省4地市乙肝及相关疾病的直接费用并分析其影响因素。方法采用整群抽样方法选择调查医院,利用问卷调查的方式获取直接费用及相关信息。采用EpiData 3.1软件双人平行录入建立数据库,采用SPSS 18.0统计软件进行分析。结果①乙肝患者年均直接费用分别为:急性乙肝19 345.77元,慢性乙肝24321.58元,乙肝后肝硬化27 761.35元,重型乙肝18 515.44元,原发性肝癌25 772.36元,其中药费所占的比例最大。②乙肝患者直接费用占家庭年收入56.86%,占人均收入205.54%,占例均GDP 68.73%。③4地市乙肝患者直接医疗费用、直接非医疗费用总体来说,差异有统计学意义(P<0.05)。④家庭月收入、是否抗病毒治疗、医保类型、疾病类型对乙肝患者年均直接费用的影响有统计学意义(P<0.05)。结论①乙肝相关疾病患者的直接费用仍很高。②乙肝相关疾病患者的直接费用对个人、家庭及社会带来了巨大的经济负担。③进一步调整收入分配,改善医疗保障制度有助于减轻乙肝患者及家庭的经济负担。
Objective To estimate the direct costs of hepatitis B and related diseases in 4 cities in Shanxi Province and analyze the influencing factors. Methods The cluster sampling method was used to select the hospital for investigation. The questionnaire was used to obtain the direct costs and related information. EpiData 3.1 software was used to establish a parallel database entry, using SPSS 18.0 statistical software for analysis. Results ① The annual average direct costs of hepatitis B patients were 19 345.77 yuan for acute hepatitis B, 24321.58 yuan for chronic hepatitis B, 27 761.35 yuan for post-hepatitis B cirrhosis, 18 515.44 yuan for severe hepatitis B and 25 772.36 yuan for primary liver cancer, The largest proportion. ② Direct costs of hepatitis B patients accounted for 56.86% of the annual household income, accounting for 205.54% of average income, accounting for 68.73% of GDP. ③ The direct medical costs and direct non-medical expenses of Hepatitis B patients in 4 cities in general were significantly different (P <0.05). ④ monthly family income, whether anti-virus treatment, health insurance type, type of disease in patients with hepatitis B average annual direct costs were statistically significant (P <0.05). Conclusion ① The direct cost of hepatitis B-related diseases is still high. ② The direct costs of hepatitis B-related diseases have caused a huge financial burden on individuals, families and the community. ③ Further adjustment of income distribution and improvement of the medical insurance system will help reduce the economic burden of hepatitis B patients and their families.
