目的:分析乳腺肿瘤的声像特征,提高乳腺良恶性肿瘤的鉴别水平,并讨论其应用价值。方法:分析36例经手术和穿刺病理证实为乳腺肿瘤的彩超声像。采用GE400型彩色超声诊断仪和10MHz高频探头,用仰卧位和侧卧位直接扫描法检查。结果:14例恶性肿瘤及22例良性肿瘤的诊断符合率分别为85.7%及72.7%。与良性肿瘤相对比,恶性肿瘤的声像图表现为:边界及轮廓显著不规则,多无包膜,内部回声显著不均匀,后壁回声减弱且不整,不清晰,呈浸润性生长,但两者在形态上差异无显著性,以圆形和椭圆形居多;血流情况表现为:恶性肿瘤的收缩期峰速明显快于良性肿瘤的收缩峰速,且恶性肿瘤的血流信号明显比良性肿瘤丰富;恶性肿瘤比良性肿瘤更常发生腋窝淋巴结肿大。结论:在检查乳腺肿瘤时,要对其声像图特征和血流参数进行综合分析,边界特征及血流参数是鉴别良恶性肿瘤的关键。检查腋窝有无淋巴结肿大,在鉴别良恶性肿瘤时有一定参考价值。
Objective: To analyze the features of breast tumors and improve the differential diagnosis of breast benign and malignant tumors, and to discuss its application value. Methods: Color Doppler echocardiography was performed in 36 cases of breast tumors confirmed by operation and puncture pathology. Using GE400-based color ultrasound diagnostic apparatus and 10MHz high-frequency probe, using supine and lateral position direct scanning method. Results: The diagnostic coincidence rates of 14 malignant tumors and 22 benign tumors were 85.7% and 72.7% respectively. Compared with benign tumors, the appearance of malignant tumors is as follows: the boundaries and contours are significantly irregular, non-enveloped, the internal echo is significantly uneven, the posterior wall echo is weakened and irregular, unclear and infiltrative, but two There was no significant difference in morphology, with a large number of round and oval; blood flow showed: the peak systolic velocity of malignant tumors was significantly faster than the peak systolic velocity of benign tumors, and malignant tumors significantly higher than the benign signal flow Tumor is abundant; Malignant tumors are more frequently seen with axillary lymph nodes than benign tumors. Conclusion: When examining the breast tumor, it is necessary to make a comprehensive analysis of its sonographic features and blood flow parameters. The boundary features and blood flow parameters are the key points in differentiating benign and malignant tumors. Check whether the axillary lymph nodes, in the identification of benign and malignant tumors have a certain reference value.
