高渗高糖非酮症昏迷(HHNC)是重型脑外伤后严重的并发症之一,病死率高达40%-70%。本文总结了对38例患者进行观察和护理的体会,认为大剂量使用脱水利尿剂是诱发HHNC的主要因素,其次,创伤及手术刺激,下丘脑损伤以及老年患者伤前并发糖尿病与HHNC的发生也密切相关,因此,密切观察病情变化,动态监测血糖,早期记录出入量和每小时尿量,及时补充大量液体,小剂量静滴胰岛素等护理措施,对降低并死率,提高治疗水平非常重要。
