脑干脓肿只占脑脓肿的1.7％。本文报告2例,均呈慢性脑干肿块表现。曾诊断为胶质瘤,其中一例术后存活。例1女性60岁。感枕部头痛数月,右面和舌麻木四周,突发右面肌下运动神经元性力弱。颅片正常,脑室造影显示第四脑室亦正常,气脑造影仅示侧脑室系统轻度扩张。脑脊液白细胞600个/立方毫米,多核占80％,蛋白6克/升,糖40毫克分子/升。8个月后复查,临床体征及气脑造影均无改变,脑脊液细胞数减少为单核细胞40个/立方毫米,蛋白8.3克/升。又一年3个月后患者突然左侧偏瘫,偏瘫前一个月曾有头痛呕吐及暂时的视力丧失发作。检查:嗜睡,视乳头水肿,有眼震,左侧软瘫,颅神经体征无改变。碘油脑室造影示导水管变细且向后及左移位。病史及检查似乎提示脑干内胶质瘤。4个月后死亡。尸检发现一个已形成完整囊壁的桥 Brain stem abscesses accounted for only 1.7% of brain abscesses.
