目的 探讨应用动态增强磁共振成像 (DCE MRI)评价的孤立性脑转移瘤血管通透性与血管内皮生长因子(VEGF)表达的空间分布相关性。方法 2 4例孤立性脑转移患者于术前行DCE MRI检查 ,分析肿瘤不同区域强化斜率 ,测定标本VEGF表达。结果 肿瘤强化呈环形 16例 ,结节状 4例 ,靶心样 4例。在环形强化中 ,肿瘤边缘呈高通透性 ,边缘VEGF高表达 12例 ;在结节状和靶心样强化中 ,边缘与中心区均表现为高通透性 ,边缘VEGF高表达为 6例 ,中心区高表达为 5例。VEGF表达与DCE MRI强化斜率间具有显著的相关性 (P <0 .0 1)。结论 DCE MRI可以用于脑转移瘤血管生成调控的监测
Objective To investigate the correlation between the vascular permeability of solitary brain metastases and the spatial distribution of vascular endothelial growth factor (VEGF) expression assessed by dynamic contrast-enhanced magnetic resonance imaging (DCE MRI). Methods Twenty-four patients with solitary brain metastases underwent DCE MRI before operation, and the enhancement slope of different regions of the tumor was analyzed to determine the expression of VEGF. Results Tumor enhancement showed a ring in 16 cases, nodules in 4 cases and bull’s eye samples in 4 cases. In the ring enhancement, the edge of the tumor was hyperpermeable and the expression of VEGF in the margin was high in 12 cases. In the nodular and bull’s-eye-like enhancement, the marginal and central regions showed hyperpermeability. High expression of the central area in 5 cases. There was a significant correlation between VEGF expression and enhancement gradient of DCE MRI (P <0.01). Conclusion DCE MRI can be used to monitor angiogenesis in brain metastases
