目的 :检测 Bcl- 2、Bax两基因蛋白在喉鳞状细胞癌 (L SCC)、喉不典型增生 (L AH)、正常喉粘膜 (L NM)中的表达 ,探讨其与喉鳞癌发生发展的关系 ,并结合临床、病理资料分析其临床意义。方法 :用免疫组织化学 S- P法检测 Bcl-2、Bax蛋白在 L SCC、L AH、L NM中的表达情况。结果 :Bcl- 2蛋白主要定位于上皮细胞的胞浆 ,少数胞膜有着色 ,呈棕黄色或黄色颗粒状 ,细胞核及癌周间质为阴性 ;Bax蛋白定位于细胞浆和细胞膜 ,间质中有少量表达 ,呈淡棕黄色或淡黄色颗粒状 ,较 Bcl- 2着色弱。在 L SCC、L AH、L NM组中 ,Bcl- 2蛋白阳性表达率分别为 5 9.46 %、6 6 .6 7%、2 0 .88% ;L SCC、L AH组与 L NM组比较有显著性差异 (P <0 .0 5 ) ;Bax蛋白阳性表达率在三组分别为 5 6 .76 %、5 0 .0 0 %、6 6 .6 7% ,三组比较无显著性差异 (P >0 .0 5 )。Spearman相关分析结果显示 :Bcl- 2、Bax蛋白表达在 L SCC中呈负相关 (r=- 0 .2 6 76 6 ,P <0 .0 5 ) ;Bcl- 2 ,Bax蛋白表达与病理分级、临床分期、淋巴结转移均无关 ,两者均仅与吸烟有关 (r=0 .35 0 5、 P <0 .0 5 ;r=- 0 .4148,P <0 .0 5 )。结论 :Bcl- 2过量表达引起的 Bcl- 2 / Bax比例失平衡在 L SCC的发生发展中起重要作用 ,Bcl- 2可以作为判断 L SCC生物学特性的参
Objective: To detect the expression of Bcl-2 and Bax in laryngeal squamous cell carcinoma (L SCC), laryngeal atypical hyperplasia (L AH) and normal laryngeal mucosa (L NM) Of the relationship, combined with clinical, pathological data analysis of its clinical significance. Methods: The expressions of Bcl-2 and Bax in L SCC, L AH and L NM were detected by immunohistochemical S-P method. Results: The Bcl-2 protein mainly localized in the cytoplasm of epithelial cells. A few of the membranous cells were stained brownish yellow or yellow granules, and the cytoplasm and interstitial cells were negative. Bax protein localized in the cytoplasm and membrane, interstitial A small amount of expression, pale brown or light yellow granular, weak color than Bcl-2. The positive rates of Bcl-2 protein in L SCC, L AH and L NM groups were 5 9.46%, 66.76% and 20.88%, respectively. There was significant difference between L SCC, L AH and L NM groups (P <0.05). The positive expression rates of Bax protein in the three groups were respectively 56.76%, 50.0% and 66.67%, there was no significant difference between the three groups P> 0 .0 5). The results of Spearman correlation analysis showed that the expressions of Bcl-2 and Bax were negatively correlated with L SCC (r = - 0.26766, P <0.05) Clinical stage and lymph node metastasis were not related, both of which were only related to smoking (r = 0.35 05, P <0.05; r = -0.4148, P <0.05). CONCLUSION: The imbalance of Bcl-2 / Bax ratio induced by overexpression of Bcl-2 plays an important role in the development of L SCC. Bcl-2 can be used as a parameter to judge the biological characteristics of L SCC
