目的 评价血清抗可溶性肝抗原/肝胰抗原抗体(抗-SLA/LP)在自身免疫性肝炎(AIH)的诊断价值.方法 收集40例AIH患者和69例对照组血清,采用酶联免疫吸附法(ELISA)和免疫印迹法(WB)检测抗-SLA/LP,比较其检测结果;分析抗-SLA/LP阳性组与阴性组临床和实验室指标的差异.结果 40例AIH患者中12例抗-SLA/LP阳性,对照组均为阴性.ELISA法和WB法的符合率为100％.抗-SLA/LP阳性的AIH患者均为女性,平均年龄明显高于阴性组[(60.92±11.97)岁vs.(48.25±10.40)岁,P=0.002].抗-SLA/LP阳性组丙氨酸氨基转移酶为(83.91±58.05)U/L、天冬氨酸氨基转移酶为(97.83±57.52)U/L,明显低于抗-SLA/LP阴性组的(367.96±415.48)U/L和(275.52±317.00)U/L(P=0.001,P=0.008).两组中均有其他自身抗体,但抗-SMA只在抗-SLA/LP阴性组中检测到.结论 抗-SLA/LP对AIH的诊断具有高度特异性,抗-SLA/LP在老年女性患者多见.
