目的:探讨血清p185因子诊断乳腺癌的特异性及敏感性。方法:选取我院2012年8月~2014年8月收治的94例乳腺癌患者及94例乳腺良性病变患者的病例资料展开回顾性分析。比较乳腺良性病变患者与乳腺癌患者的血清p185因子水平并作血清p185因子水平预测乳腺癌的ROC模型获得敏感性、特异性及最佳截断值。探析血清p185因子与乳腺癌患者临床病理特征的关系并分析血清p185因子与预后的相关性。结果:乳腺癌患者血清p185因子水平为(7.09±1.34)ng·ml~(-1),高于乳腺良性病变患者的(6.21±1.11)ng·ml~(-1)(P<0.05)。当血清p185因子水平>6.83 ng·ml~(-1)时,预测乳腺癌的敏感性为0.9、特异性为0.95。乳腺癌患者中肿瘤直径>2.0 cm、临床分期为(Ⅲ+Ⅳ)期乳腺癌患者中血清p185阳性所占百分率明显高于阴性。1年及2年生存情况与血清p185阴阳性之间均存在明显相关性(r=0.442,P=0.002;r=0.518,P=0.000)。结论:血清p185因子诊断乳腺癌的敏感性与特异性均较高,且血清p185因子水平与患者肿瘤大小、临床分期有一定相关性,还可用于预后预测,具有较大临床价值。
Objective: To investigate the specificity and sensitivity of serum p185 in the diagnosis of breast cancer. Methods: A retrospective analysis of 94 cases of breast cancer and 94 cases of benign breast lesions admitted to our hospital from August 2012 to August 2014 were retrospectively analyzed. To compare the serum level of p185 in patients with benign breast disease and breast cancer and obtain the sensitivity, specificity and optimal cutoff value for ROC model of breast cancer with serum level of p185. To explore the relationship between serum p185 factor and clinicopathological features of breast cancer and to analyze the correlation between serum p185 and prognosis. Results: The serum level of p185 in breast cancer patients was (7.09 ± 1.34) ng · ml -1, higher than that in benign breast lesions (6.21 ± 1.11) ng · ml -1 (P <0.05). When the serum level of p185 was> 6.83 ng · ml -1, the sensitivity of breast cancer prediction was 0.9 and the specificity was 0.95. Breast cancer in patients with tumor diameter> 2.0 cm, clinical stage (Ⅲ + Ⅳ) breast cancer patients with serum p185 positive percentage was significantly higher than negative. There was a significant correlation between 1-year and 2-year survival and serum p185 y-positivity (r = 0.442, P = 0.002; r = 0.518, P = 0.000). Conclusions: The sensitivity and specificity of serum p185 factor in the diagnosis of breast cancer are high. The serum level of p185 is correlated with the tumor size and clinical stage. It may be used in prognosis prediction and has great clinical value.
