目的探讨分化型甲状腺癌的外科治疗及影响其复发的因素。方法选取2008年1月～2012年1月我院收治的分化型甲状腺癌患者52例,研究外科治疗方案和效果,分析影响其复发的因素。结果在我院行首次手术治疗36例中,复发2例,复发率为5.6%;非我院首治的14例中,病理证实复发共2例,复发率为14.3%。对4例复发性DTC的相关因素分析结果表明,首次治疗术式对复发影响有显著性(P<0.05);性别、年龄、病理类型、病灶大小、疾病分期、淋巴结转移情况等均对复发影响无显著性(P均>0.05)。结论对原发灶局限于一侧腺叶的DTC患者,单侧腺叶+峡部切除术/颈淋巴结清扫是首次手术治疗的有效方式。首次治疗术式是影响DTC复发的重要因素。
Objective To investigate the surgical treatment of differentiated thyroid carcinoma and its influencing factors. Methods Fifty-two patients with differentiated thyroid cancer admitted to our hospital from January 2008 to January 2012 were enrolled in this study. The surgical regimen and effect were studied, and the factors influencing recurrence were analyzed. Results The first surgery in our hospital 36 cases, 2 cases of recurrence, the recurrence rate was 5.6%; non-hospital first rule in 14 cases, the pathology confirmed a total of 2 cases of recurrence, the recurrence rate was 14.3%. The analysis of the related factors of the 4 recurrent DTCs showed that the first-time operation had a significant effect on recurrence (P <0.05). The gender, age, pathological type, size of the lesion, stage of the disease, lymph node metastasis, No significant (P> 0.05). Conclusions For patients with DTC whose primary tumor is localized to the side of the gland lobe, unilateral lobectomy + isthmicctomy / dissection of the cervical lymph nodes is an effective first-line surgical treatment. The first treatment is an important factor affecting the recurrence of DTC.
