目的:分析乳腺导管内癌(DCIS)的超声声像图特征及超声对该病的诊断价值。方法:回顾分析53例病理确诊的乳腺导管内癌超声图像,包括腺体结构及导管扩张情况及肿块形态、大小、边界、边缘、内部回声、钙化、回声衰减、血流分布及腋窝淋巴结情况。结果:DCIS超声表现共分为实性肿块伴钙化、单纯低回声结节、导管扩张填充或囊实性肿块、结构紊乱或单纯钙化、无特殊表现5型,各型分别检出为16、8、13、11、5例,前4型超声诊断符合率分别为93.8%、37.5%、76.9%和81.8%,假阴性率9.4%。结论:DCIS超声表现多样,不同类型的超声诊断符合率存在明显差异。对实性肿块伴钙化型超声诊断价值最高,单纯低回声结节型最低,存在一定比例的假阴性。
Objective: To analyze the ultrasonographic features of breast ductal carcinoma of the breast (DCIS) and the diagnostic value of ultrasound in the diagnosis of the disease. Methods: Fifty-three cases of pathologically diagnosed breast intraductal carcinoma were retrospectively analyzed, including the structure of the gland, the dilatation of the duct and the morphology, size, boundary, margins, internal echo, calcification, echo attenuation, blood flow distribution and axillary lymph node status. Results: DCIS was divided into solid tumors with calcification, simple hypoechoic nodules, duct dilatation and filling or cystic solid tumors, structural disorders or simple calcification, with no special type 5, and each type was found to be 16,8 , 13, 11, and 5 cases. The coincidence rates of the former 4 ultrasound diagnosis were 93.8%, 37.5%, 76.9% and 81.8% respectively, and the false negative rate was 9.4%. CONCLUSION: The performance of DCIS is diverse, and the coincidence rates of different types of ultrasound diagnosis are obviously different. The real mass with calcified ultrasound diagnosis of the highest value, the lowest hypoechoic nodules, there is a certain percentage of false negatives.
