目的 :研究鼻咽癌 (NPC)病人放疗后听觉诱发电位改变的原因。方法 :48例鼻咽癌病人在放疗前、放疗中(鼻咽部 DT40~ 48Gy)、放疗结束、放疗结束后 3个月、 6个月、 12个月进行脑干听觉诱发电位 (BAEP)的检测。用单因素及多因素 COX模型分析 BAEP改变原因。结果 :鼻咽癌放疗后 12个月内 BAEP总异常率为 6 4. 6 % (31/ 48) ,放疗中(鼻咽部 DT40~ 48Gy)、放疗结束以及放疗结束后 3个月、 6个月、 12个月异常率分别为 16 . 7% (8/ 48) ,2 9. 8% (14/47) ,2 5 . 0 % (12 / 48) ,5 2 . 2 % (2 4/ 46 ) ,5 2 . 3% (2 3/ 44 )。年龄≤ 35岁 ,36~ 5 0岁 ,>5 0岁分别为 2 5 % (l/ 4) ,5 6 . 0 %(14/ 2 5 ) ,84. 2 % (16 / 19) ,差异有显著性 (P =0 . 0 3)。两耳前野组异常率为 5 3. 1% (17/ 32 ) ,面颈联合野组者 87. 5 %(14/ 16 ) ,后者明显比前者高 ,差异有显著性 (P =0 . 0 2 )。脑干 (前缘 )受量 45~ 5 l Gy者 BAEP异常率为 5 8. 6 % (17/2 9) ,5 2~ 6 l Gy者为 73. 7% (14/ 19) ,两者相比未见显著性差异 (P =0 . 2 9)。多因素分析显示影响 BAEP改变的有照射面积、年龄。结论 :鼻咽癌放疗后 BAEP改变与年龄 ,照射面积有关
Objective: To study the causes of the change of auditory evoked potentials in patients with nasopharyngeal carcinoma (NPC) after radiotherapy. Methods: Forty-eight patients with nasopharyngeal carcinoma underwent radiotherapy (DT40 ~ 48Gy), end of radiotherapy, 3 months, 6 months and 12 months after radiotherapy. The brainstem auditory evoked potential (BAEP) Detection. Univariate and multivariate COX models were used to analyze the causes of BAEP changes. Results: In the 12 months after radiotherapy of nasopharyngeal carcinoma, the total abnormal BAEP was 66.6% (31/48), during radiotherapy (nasopharyngeal DT40-48Gy), after the end of radiotherapy and 3 months after radiotherapy, 6 Month and 12 months respectively, the abnormal rates were 16.7% (8/48), 29.8% (14/47), 25.0% (12/48), 52.2% (2 4 / 46), 52.3% (23/44). The patients were 35 years old, 36 to 50 years old and> 50 years old, respectively, with 25% (l / 4), 56.0% (14/2 5) and 84.2% (16/19) Significance (P = .0 3). The rate of abnormalities in the two groups was 53.1% (17/32) in the two groups and 87.5% (14/16) in the face-neck joint group. The latter group was significantly higher than the former (P = 0. 0 2). The BAEP abnormalities were 45.6% (17/2 9) and 73.7% (14/19) in the brain stem (frontal) with 45-5 l Gy, respectively There was no significant difference (P = 0.29). Conclusion: The change of BAEP after radiotherapy of nasopharyngeal carcinoma is related to age and irradiation area
