目的检测子宫内膜癌中丝裂原活化蛋白激酶4(MAP2K4)和波形蛋白的表达,并分析其表达与子宫内膜癌的病理特征及预后的关系。方法采用免疫组化SP法检测128例子宫内膜癌组织中MAP2K4和波形蛋白的表达,并分析MAP2K4和波形蛋白的表达与子宫内膜癌的病理特征的相关性。结果子宫内膜癌组织中MAP2K4蛋白和波形蛋白的阳性表达率分别为49/128(38.3%)和83/128(64.8%),差异有统计学意义(P<0.001)。MAP2K4蛋白的阳性表达与肿瘤的FIGO分期及淋巴结是否转移呈负相关(P=0.010,P=0.016),而波形蛋白的阳性表达与肿瘤的FIGO分期(P=0.025)、组织学分级(P=0.017)、肌层浸润深度(P=0.044)及淋巴结是否转移(P=0.032)呈正相关。在子宫内膜癌中,MAP2K4和波形蛋白的表达呈负相关(r=-0.598,P<0.001)。MAP2K4蛋白阳性表达(MAP2K4+)的患者总体生存率较其阴性表达(MAP2K4-)的患者高(P=0.002),而波形蛋白阳性表达的患者总体生存率较其阴性表达的患者低(P=0.007)。并且,MAP2K4+/波形蛋白阴性患者的无病生存时间最长,而MAP2K4-/波形蛋白阳性患者的无病生存时间最短(P=0.004)。结论 MAP2K4和波形蛋白的联合检测有助于子宫内膜癌的早期诊断及预后分析。
Objective To detect the expression of mitogen - activated protein kinase 4 (MAP2K4) and vimentin in endometrial carcinoma and analyze its relationship with the pathological features and prognosis of endometrial carcinoma. Methods The expression of MAP2K4 and vimentin in 128 cases of endometrial carcinoma was detected by immunohistochemical SP method. The correlation between the expression of MAP2K4 and vimentin and the pathological features of endometrial carcinoma was analyzed. Results The positive rates of MAP2K4 protein and vimentin in endometrial carcinoma were 49/128 (38.3%) and 83/128 (64.8%) respectively, with statistical significance (P <0.001). The positive expression of MAP2K4 protein was negatively correlated with FIGO staging and lymph node metastasis (P = 0.010, P = 0.016), while the positive expression of vimentin was positively correlated with FIGO stage (P = 0.025), histological grade 0.017), depth of myometrial invasion (P = 0.044) and lymph node metastasis (P = 0.032). In endometrial carcinoma, MAP2K4 and vimentin expression was negatively correlated (r = -0.598, P <0.001). The overall survival rate of patients with MAP2K4 positive expression (MAP2K4 +) was significantly higher than that of patients with negative MAP2K4 expression (P = 0.002), while the overall survival rate of patients with positive expression of vimentin was lower than that with negative expression (P = 0.007 ). Also, patients with MAP2K4 + / vimentin-negative disease had the highest disease-free survival time, while MAP2K4- / vimentin-positive patients had the lowest disease-free survival time (P = 0.004). Conclusions The combined detection of MAP2K4 and vimentin contributes to the early diagnosis and prognosis analysis of endometrial cancer.
